The development of opportunistic infections due to poor denture hygiene conditions justified the search for effective hygiene protocols for controlling denture biofilm. Objective This study evaluated Ricinus communis and sodium hypochlorite solutions in terms of biofilm removal ability, remission of candidiasis, antimicrobial activity, and participant satisfaction.Material and Methods It was conducted a controlled clinical trial, randomized, double-blind, and crossover. Sixty-four denture wearers with (n=24) and without candidiasis (n=40) were instructed to brush (3 times/day) and immerse their dentures (20 min/day) in different storage solutions (S1 / S2: 0.25% / 0.5% sodium hypochlorite; S3: 10% R. communis; S4: Saline).The trial period for each solution was seven days and a washout period of seven days was used before starting the use of another solution. The variables were analyzed at baseline and after each trial period. The biofilm of inner surfaces of maxillary dentures was disclosed, photographed, and total and dyed areas were measured (Image Tool software). The percentage of biofilm was calculated. Remission of candidiasis was assessed by visual scale and score were attributed. Antimicrobial activity was assessed by the DNA-Checkerboard hybridization method. Patient satisfaction was measured using a questionnaire.Results S1 (4.41±7.98%) and S2 (2.93±5.23%) were more effective then S3 (6.95±10.93%) in biofilm remotion(P<0.0001). All solutions were different from the control (11.07±11.99%). S3 was the most effective solution in remission of candidiasis (50%), followed by S1 (46%). Concerning antimicrobial action, S1/S2 were similar and resulted in the lowest microorganism mean count (P=0.04), followed by S3. No significant differences were found with patient’s satisfaction.Conclusions 10% R. communis and 0.25% sodium hypochlorite were effective in biofilm removal, causing remission of candidiasis and reducing the formation of microbial colonies in denture surfaces. All solutions were approved by patients.

ResumoObjetivo: O objetivo deste estudo foi avaliar a relação entre fatores funcionais e qualitativos no desenvolvimento da estomatite protética (EP) (de acordo com a classificação de Newton) em usuários de dentadura acrílica residentes no norte do Brasil. Material e método: Um total de 99 pacientes, que usavam dentadura superior de resina acrílica parcial ou total, foi incluído neste estudo. Os participantes preencheram um formulário de dados epidemiológicos, que incluiu gênero, idade, fatores locais (hábitos de higiene, retirar a dentadura ao dormir, uso de colutório, condição atual da dentadura, idade da dentadura) e fatores funcionais (dimensão vertical de repouso, dimensão vertical de oclusão, oclusão, retenção, estabilidade estática e dinâmica). Para detectar leveduras, amostras foram coletadas da superfície interna da dentadura e da mucosa palatal em contato com esta. Posteriormente, as amostras foram cultivadas em ágar Sabouraud dextrose, observando-se características macro e microscópicas. Resultado: No presente estudo, não foi encontrada relação significante entre gênero e início da doença. Baseada na classificação de Newton, 36,3% dos pacientes apresentaram EP e 89,0% foram colonizados por leveduras; destes indivíduos, 50% tiveram lesões tipo I, 33,3% tiveram lesões tipo II e 16,6% tiveram lesões tipo III. Todos os fatores locais e qualitativos, exceto o uso de colutório, foram clinicamente relevantes para o desenvolvimento da doença. Conclusão: Estomatite protética em usuários de dentadura do norte do Brasil foi multifatorial, associando fatores locais, funcionais e microbiológicos.Descritores: Dentaduras; estomatite; higiene bucal; Candida. DCT in Restorative Dentistry IntroductionIn order to achieve optimal oral health it is well documented in the literature that denture wearing patients should be advised to maintain high standards of both oral hygiene and denture hygiene. [1][2][3][4][5][6] Dentures can accumulate plaque and develop calculus deposits in a manner similar to natural teeth, and the composition of denture plaque differs when compared with dental plaque. Studies have shown that denture plaque can contain a number of potentially harmful microorganisms including: Methicillinresistant Staphylococcus aureus, Candida albicans, and Steptococcus mutans. 7-11Previous studies indicate that inadequate denture hygiene can lead to an increase in accumulation of denture plaque, which can then increase the risk of developing oral and dental diseases such as dental decay, periodontal disease, and denture stomatitis. 4,[10][11][12][13] Unfortunately many patients have been shown to have inadequate denture hygiene and some still wear their dentures at night, despite evidence that nocturnal denture wearing is linked to a decrease in oral health and provides no benefit to patients' quality of sleep. 5,12,14,15 A number of different cleaning regimes are available for patients to clean their dentures and they can be classified into either chemical or mechanical methods. According to a number of different studies and surveys evaluating the denture hygiene habits of patients, the most commonly used mechanical method is a toothbrush with soap and water or toothpaste, while the most commonly used chemical method is either sodium hypochlorite-based cleaner or a peroxide-based cleaner. 1,2,5,14,16,17 Evidence from studies conducted outside the UK provides evidence of the denture care habits and the quality of denture hygiene of non-UK cohorts. Their results may potentially be used to inform on the denture care advice given to UK denture wearers, however, there likely will be potential differences in the availability of denture care materials and in patient denture care/hygiene attitudes. Few studies and surveys have been conducted in the UK which outline the quality of patients' denture hygiene, their denture cleaning habits, and the quality of clinical record keeping.The purpose of this article is to outline the results of a clinical audit conducted in the prosthodontics department of a regional dental hospital. It follows on from work conducted within general dental practice and seeks to determine whether denture hygiene differs between a primary care and secondary care cohort; this comparison has yet to be published in the available literature.
