Robotic pyelolithotomy in a congenital pelvic kidney: side docking and robotic prostatectomy port - site approach

Abstract: Introduction and Objectives Ectopic pelvic kidneys with renal stones are challenging to treat. We report our experience in managing a case of ectopic pelvic kidney with a pelvic stone by robotic pyelolithotomy after failure of flexible ureteroscopy.Materials and Methods A 46-year old male with 2 months history of vague lower abdominal pain was found to have on Computed Tomography scan a left ectopic pelvic kidney with a 12mm stone in an anomalous renal pelvis. Flexible ureteroscopy failed to reach the stone tw… Show more

“…A total of 249 RASs were performed in Kuwait until June 2018, with 204 cases performed at the SAUC, and 45 performed at the Chest Disease Hospital. Different types of urological RAS were performed between February 2014 and June 2016 including: RARP, robot-assisted radical nephrectomy (RARN), RAPN, robot-assisted pyeloplasty (RAP), robot-assisted adrenalectomy (RAA), and pyelolithotomies that failed ureteroscopic management [16][17][18][19] (Figure 3). All procedures were performed transperitoneally, and no case required open conversion or received a blood transfusion, with a median hospital stay of 3 days and no major complications ( Figure 3).…”

“…One article mentioned the use of the fingers of a sterile glove as retrieval bags (21). Interestingly, the use of a flexible cystoscope has also been described to ease intraperitoneal stone removal after pyelolithotomy is closed (22).…”

“…In the pediatric population, this would be comparable to positioning and port placement for bilateral vesicoureteral reimplantations. A transmesenteric approach was utilized for endopyelotomy and subsequent pyelolithotomy thus avoiding mobilization of the colon and its increased operative time and potential risk for injury (22, 25).…”

“… ABSTRACT
Introduction: The management of urolithiasis ectopic pelvic kidneys (EPK) can be challenging because of the aberrant anatomy ( 1 4 ). We demonstrate the step-by-step technique of the laparoscopic approach for treating urolithiasis in EPK.
