OBJECTIVE To characterize the activities of clinical nature developed by pharmacists in basic health units and their participation in educational activities aiming at health promotion.METHODS This article is part of the Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos – Serviços, 2015 (PNAUM – National Survey on Access, Use and Promotion of Rational Use of Medicines – Services, 2015), a cross-sectional and exploratory study, of evaluative nature, consisting of a survey of information in a representative sample of cities, stratified by the Brazilian regions that constitute domains of study, and a subsample of primary health care services. The interviewed pharmacists (n=285) were responsible for the delivery of medicines and were interviewed in person with the use of a script. The characterization of the activities of clinical nature was based on information from pharmacists who declared to perform them, and on participation in educational activities aiming at health promotion, according to information from all pharmacists. The results are presented in frequency and their 95% confidence intervals.RESULTS From the interviewed subjects, 21.3% said they perform activities of clinical nature. Of these, more than 80% considered them very important; the majority does not dispose of specific places to perform them, which hinders privacy and confidentiality in these activities. The main denominations were “pharmaceutical guidance” and “pharmaceutical care.” The registration of activities is mainly made in the users’ medical records, computerized system, and in a specific document filed at the pharmacy, impairing the circulation of information among professionals. Most pharmacists performed these activities mainly along with physicians and nurses; 24.7% rarely participated in meetings with the health team, and 19.7% have never participated.CONCLUSIONS Activities of clinical nature performed by pharmacists in Brazil are still incipient. The difficulties found point out to the professionals’ improvisation and effort. The small participation in educational activities of health promotion indicates little integration of pharmacists with the health team and of pharmaceutical services with other health actions.

Patient prioritization in comprehensive medication management services allows coordinating care and guiding patients according to their clinical profile and their medication use. The aim of the study is to identify and describe factors that indicate the need for comprehensive medication management services among primary care hypertension patients within a public health system from the perspective of patients, pharmacists, nurses and physicians. A qualitative study was carried out with interviews with nurses, pharmacists and physicians (n = 20), and two focus groups with hypertensive patients (n = 12) at primary health care facilities and a public outdoor fitness area between January and February 2019 in Brazil. All interviews were transcribed and analyzed using the Atlas.ti® software. The data analysis revealed the following factors indicative of the need to refer hypertension patients to a pharmacist: lifestyle habits, comorbidities, health care utilization and medication use. The issues identified and the information obtained from the qualitative research and compared with literature studies reviewed allowed defining dimensions that should be considered as an aid in the selection of uncontrolled hypertensive patients for comprehensive medication management services.

Introducción: según estudios recientes, los Servicios Profesionales Farmacéuticos Asistenciales (SPFA) demostraron sus beneficios en la calidad de vida de pacientes y sistemas de salud. Los SPFA son actividades sanitarias prestadas desde la farmacia comunitaria (FC) para la prevención de enfermedades y la mejora de la salud mediante la optimización del uso y del resultado de los tratamientos. Sin embargo, en España existe un bajo porcentaje de farmacias comunitarias que ofrecen SPFA, a excepción de la dispensación, indicación farmacéutica, medida de parámetros clínicos, la educación/información sanitaria y la formulación magistral, ya arraigados en la profesión desde hace tiempo.Objetivo: analizar los factores facilitadores y barreras en la demanda de determinados SPFA por parte de usuarios potenciales, desde el enfoque del producto según las 4Ps del Marketing Mix.Material y métodos: estudio cualitativo de tipo explicativo-interpretativo y exploratorio basado en la teoría fundamentada, mediante análisis inductivo aplicado a entrevistas semi-estructuradas, individuales y grupales.Resultados: las barreras y facilitadores son similares en todos los SPFA analizados, salvo algunas excepciones. Estas barreras y facilitadores son multifactoriales, porque están vinculados con la farmacia, el usuario, factores externos y el farmacéutico.Conclusiones: las barreras en la demanda de los SPFA son la baja adaptación a las necesidades del usuario, los nombres poco entendibles, la falta de integración de la FC en el sistema sanitario, el desconocimiento sobre los SPFA, las insuficientes evidencias de éxito y la satisfacción en servicios percibidos como similares realizados fuera de la FC. Por el contrario, los facilitadores que ayudan al aumento de la demanda de los SPFA son la actitud y aptitud del farmacéutico, el buen trato al paciente, la accesibilidad, cercanía y confortabilidad de la FC y la necesidad del paciente por mejorar su estado de salud.
