Objectives: To evaluate factors related to dangerousness cessation at the end of involuntary commitment based on an analysis of expert reports. In light of the current legal requirement of dangerousness cessation as a pre-requisite for prison or internment release of individuals subjected to the safety measure, we sought elements to reflect on the practice of expert examiners in charge of making this decision. Methods: The authors revised 224 expert psychiatric dangerousness cessation reports released 2011 through 2014 and collected data for a statistical analysis. Results: The following variables were associated with positive risk cessation assessments: no inadequate behavior (according to the assistant professionals), no productive psychotic symptoms, no negative symptoms, presence of insight, presence of a support network, and no psychoactive substance abuse. The following variables were associated with negative dangerousness cessation decisions: early onset of malfunction, lack of insight, negative attitudes, active signs of major mental illness, presence of impulsiveness, poor response to treatment, presence of plans lacking feasibility, exposure to destabilizing factors, lack of personal support, and presence of stress. Conclusions: In this study we were able to identify factors associated with dangerousness in a sample of expert reports. The knowledge of factors linked to a higher risk of recidivism in illegal activities or violent behavior is crucial for decision-making regarding the release of offenders after their legally established period of involuntary commitment.

Los sistemas judiciales exigen a los psicólogos evaluar a sujetos que cumplen condenas en establecimientos penitenciarios con el fin de valorar la posibilidad que estos puedan reinsertarse anticipadamente a la sociedad. Para ello, los psicólogos penitenciarios han desarrollado técnicas y procedimientos de evaluación del riesgo de la conducta violenta basadas en la identificación de factores de riesgo. El objetivo de este estudio fue adaptar el primer instrumento de evaluación del riesgo de violencia al contexto penitenciario peruano. Se estableció la validez de contenido por juicio de expertos, con una muestra de 303 internos. La validez de constructo fue establecida a través del análisis factorial exploratorio y confirmatorio. Por último, se estableció la precisión diagnóstica, por medio de la curva ROC. Los resultados indican que la adaptación de la EVCV-RR es un instrumento adecuado para valorar el riesgo de criminógeno. Adaptation of the Evaluative Scale of Violent Behavior and Risk of Recidivism (SVVB-RR) in a Peruvian penitentiary sample The judicial systems require psychologists to evaluate subjects who serve sentences in penitentiary establishments in order to assess the possibility of reinsertion in society. Consequently, prison psychologists have developed techniques and procedures for assessing the risk of violent behavior based on the identification of risk factors. The objective of this study was to adapt the first instrument for assessing the risk of violence in the Peruvian prison context. Content validity was evidenced by expert judgment and with a sample of 303 inmates, construct validity was identified through exploratory and confirmatory factor analysis. Finally, the diagnostic accuracy was established by means of the ROC curve.

The assessment and management of risk for future violence is a core requirement of mental health professionals in many settings. Despite an increasing need for violence risk assessments across diverse contexts, little is known regarding the ecological validity of many widely used risk assessment schemes or the level of reliability with which actual practicing clinicians score these instruments. The current study investigated the interrater reliability of the Historical, Clinical, and Risk Management-20 (HCR-20), a widely used structured professional tool to assess violence risk, among 21 practicing clinicians in a forensic psychiatric program in Ontario, Canada. Results suggest that clinicians with varying professional training backgrounds and experience were able to rate the HCR-20 with good to excellent levels of reliability across three patients who varied in risk level. Consistent with studies investigating rater reliability for research purposes, we found that the risk management scale of the HCR-20 was the most challenging for clinicians to rate reliably. Importantly, results from generalizability theory analyses revealed that less than 3% of the variance in HCR-20 total scores and summary risk ratings is attributable to rater effects, whereas the majority of variance is attributable to differences among patients.
