We studied whether readiness to change predicts alcohol consumption (drinks per day) 3 months later in 267 medical inpatients with unhealthy alcohol use. We used 3 readiness to change measures: a 1 to 10 visual analog scale (VAS) and two factors of the Stages of Change Readiness and Treatment Eagerness Scale: Perception of Problems (PP) and Taking Action (TA). Subjects with the highest level of VAS-measured readiness consumed significantly fewer drinks 3 months later [Incidence rate ratio (IRR) and 95% confidence interval (CI): 0.57 (0.36, 0.91) highest vs. lowest tertile]. Greater PP was associated with more drinking [IRR (95%CI): 1.94 (1.02, 3.68) third vs. lowest quartile]. Greater TA scores were associated with less drinking [IRR (95%CI): 0.42 (0.23, 0.78) highest vs. lowest quartile]. Perception of Problems' association with more drinking may reflect severity rather than an aspect of readiness associated with ability to change; high levels of Taking Action appear to predict less drinking. Although assessing readiness to change may have clinical utility, assessing the patient's planned actions may have more predictive value for future improvement in alcohol consumption.

Este estudo apresenta a validação do University of Rhode Island Change Assessment (URICA) para usuários de substâncias psicoativas ilícitas no Brasil. A amostra foi de 214 sujeitos dos sexos masculino (n=194) e feminino (n=20), com faixa etária entre 13 e 44 anos de idade (M=22,93; DP=7,94). A coleta ocorreu em Porto Alegre (RS), em ambulatórios para tratamento da dependência química (n=89) e locais de internação (n=125). As análises estatísticas evidenciaram uma boa consistência interna da escala de 24 itens (a=0,657). A partir de análises estatísticas foi construído o escore T, realizando a normatização brasileira da URICA para drogas ilícitas, que apresentou bons resultados psicométricos, podendo ser usada em estudos que proponham investigar a motivação para mudança de comportamentos-problema.

This case study presents some factors involved in modulating the behaviors of a drug using adolescent. Therefore, the adolescent’s and a family member’s levels of self-efficacy and hope were assessed, whereas both received a brief intervention for adolescent drug use with parent sessions. The adolescent’s drug use and motivation were also investigated. The main results indicate that the adolescent showed less motivation at follow-up concerning actions, although his ambivalence and his awareness regarding drug use problems increased. The adolescent lapsed back to tobacco use, albeit presented a structured routine at 14 months after the posttest. Based on the results, when considering the importance to improve positive aspects in adolescence, in order to do so, their caretakers should also be supported.
