BackgroundHigh resting heart rate is considered an important factor for increasing mortality chance in adults. However, it remains unclear whether the observed associations would remain after adjustment for confounders in adolescents.ObjectivesTo analyze the relationship between resting heart rate, blood pressure and pulse pressure in adolescents of both sexes.MethodsA cross-sectional study with 1231 adolescents (716 girls and 515 boys) aged 14-17 years. Heart rate, blood pressure and pulse pressure were evaluated using an oscillometric blood pressure device, validated for this population. Weight and height were measured with an electronic scale and a stadiometer, respectively, and waist circumference with a non-elastic tape. Multivariate analysis using linear regression investigated the relationship between resting heart rate and blood pressure and pulse pressure in boys and girls, controlling for general and abdominal obesity.ResultsHigher resting heart rate values were observed in girls (80.1 ± 11.0 beats/min) compared to boys (75.9 ± 12.7 beats/min) (p ≤ 0.001). Resting heart rate was associated with systolic blood pressure in boys (Beta = 0.15 [0.04; 0.26]) and girls (Beta = 0.24 [0.16; 0.33]), with diastolic blood pressure in boys (Beta = 0.50 [0.37; 0.64]) and girls (Beta = 0.41 [0.30; 0.53]), and with pulse pressure in boys (Beta = -0.16 [-0.27; -0.04]).ConclusionsThis study demonstrated a relationship between elevated resting heart rate and increased systolic and diastolic blood pressure in both sexes and pulse pressure in boys even after controlling for potential confounders, such as general and abdominal obesity.

BackgroundPrecise blood pressure (BP) measurements are central for the diagnosis of hypertension in clinical and epidemiological studies. The purpose of this study was to quantify the variability in BP associated with arm side, body position, and successive measurements in the setting of a population-based observational study. Additionally, we aimed to evaluate the influence of different measurement conditions on prevalence of hypertension.MethodsThe sample included 967 men and 812 women aged 45 to 83 years at baseline. BP was measured according to a standardized protocol with oscillometric devices including three sitting measurements at left arm, one simultaneous supine measurement at both arms, and four supine measurements at the arm with the higher BP. Hypertension was defined as systolic BP (SBP) ≥140 mmHg and/or diastolic BP (DBP) ≥90 mmHg. Variability in SBP and DBP were analysed with sex-stratified linear covariance pattern models.ResultsWe found that overall, no mean BP differences were measured according to arm-side, but substantial higher DBP and for men also higher SBP was observed in sitting than in supine position and there was a clear BP decline by consecutive measurement. Accordingly, the prevalence of hypertension depends strongly on the number and scheme of BP measurements taken to calculate the index values.ConclusionsThus, BP measurements should only be compared between studies applying equal measurement conditions and index calculation. Moreover, the first BP measurement should not be used to define hypertension since it overestimates BP. Embora o exercício isométrico esteja sendo usada para redução da pressão arterial (PA). Os estudos são divergentes quantos aos efeitos agudos, que é uma medida de risco cardiovascular, além de não ser conhecido os efeitos da intensidade do exercício.Analisar as respostas cardiovasculares aguda após exercício isométrico com handgrip com diferentes intensidades. Fizeram parte deste estudo cross-over, 23 homens saudáveis, os quais foram submetidos, a três sessões experimentais: 4x2 minutos de contração a 30% (S30) e 50% (S50) da contração voluntária máxima e controle (SC). A PA e os parâmetros da variabilidade da frequência cardíaca do domínio do tempo (SDNN, RMSSD e PNN50) e da frequência (LF, HF e LF/HF) foram obtidos antes e após 30 minutos das sessões. Não foram observadas diferenças significantes na PA sistólica (S50:115±9vs.117±9 mmHg; S30:112±10vs.114±9 mmHg; SC:115±9vs.113±8 mmHg, p>0,05) e diastólica (S50:61±5vs.65±5 mmHg; S30:60±4vs.63±6 mmHg; SC:62±8vs.63±8 mmHg, p>0,05). Após a S50, houve menor aumento do PNN50 (S50:+1±5; S30:+7±7;SC:+6±8%, p=0,012) e maior aumento do LF/HF (S50:+1,59±2,55;S30:-0,49±1,84;SC:+0,37±1,49, p=0,049) comparado as demais sessões. Após a realização do exercício isométrico, não houve alterações pressóricas, enquanto que o exercício com maior intensidade gerou aumento da modulação simpática e redução da modulação parassimpática após exercício.
