A assistência domiciliar privada em saúde e suas estratégias (aparentes e ocultas)

Abstract: This paper analyses the findings of an investigation into private home healthcare. Considered the most visible model of Managed Healthcare, it is being developed through the supply of multiple services. This concept is based on a set of principles - out-of-hospital care, lower infection risks, family-based benefits for patients - all linked to a cost-cutting strategy. Some labor relations aspects are also discussed that underpin the development of this type of care.

“…In this operator, HC was regarded as an alternative to improve the quality of care while reducing costs, confirming the logic previously described in other HC studies (Silva et al, 2012(Silva et al, , 2010Cunha, 2007;Franco and Merhy, 2008;Rehem and Trad, 2005;Santos et al, 2011) Operator #3 has been offering HC since 2004 as a structured program, which is part of their list of services. The implementation of the program was mainly motivated by the demands of families who might not adjust to situations demanding continuous patient care after hospital discharge.…”

“…(9) In this sense, the teams use user classification scores, based on criteria of functional capacity, and above all on the history of spending on hospital services. It is the nurse's responsibility, after receiving the request for the user's admission, or the user him/herself promotes capture of hospitalization units to proceed with classification, according to the score adopted in the service, revealing that the action of this professional contributes to the rationalization of costs.…”

“…(1,(7)(8) Moreover, there is evidence that the offer of this care modality responds to the movement of managed health care into the logic of management and financial rationality represented by the reduction in the costs of procedures, making the productive processes more flexible, and leading to precarious working relationships and conditions. (9) When considering the extension and problems surrounding supplemental home health care services it is pertinent for us to reflect on the inclusion of the work of nursing in the care modalities.…”

“…1,6 However, new needs deriving from population aging, demands for better quality of care and for integral and continuous care have strengthened anti-hegemonic practices and the formulation of new strategies and mechanisms for healthcare. Thus, it is possible to observe the emergence and amplification of non-traditional assistance spaces, such as Estratégia Saúde da Família (Family Health Strategy), therapeutic home care services targeted at HIV/AIDS patients, proposals for palliative home care, among others.…”
