Dermoscopy of Bowen's disease: pigmented variant on the penis

Abstract: Pigmented Bowen's disease (PBD) is a variant of squamous cell carcinoma in situ and represents less than 2% of cases of Bowen's disease. It is characterized by a sharply demarcated, pigmented plaque with a scaly or crusted surface on intertriginous and genital areas. The authors describe a case of PBD on the penis and analyze the dermoscopic aspects of this type of lesion. Keywords: Bowen's disease; Carcinoma, squamous cell; Dermoscopy; Neoplasms, squamous cell Resumo: A doença de Bowen Pigmentada (DBP) é uma … Show more

“…Dermoscopic signs previously documented for Bowenoid papulosis include an unspecified pattern, pigmentation (brown‐grey dots aligned in a linear distribution), dotted vessels and a papillomatous or keratotic surface . The documented cases of penile Bowen disease have all been of the pigmented subtype . Cases exhibited pigmentation (brown, grey dots and granules) and structureless areas (blue‐black, hypopigmented, pink‐grey or skin‐coloured) .…”

“…The dermoscopy of PeIN has received scant attention. A review of the literature revealed documentation of features for only four cases of Bowenoid papulosis and four cases of pBD affecting the penis (Table ) . Dermoscopic signs previously documented for Bowenoid papulosis include an unspecified pattern, pigmentation (brown‐grey dots aligned in a linear distribution), dotted vessels and a papillomatous or keratotic surface .…”

“…Genital BD usually is found in elderly men, especially on the mucosa of the penis (glans or prepuce). Some authors consider mucosal BD equal to the erythroplasia of Queyrat; however, some of them accept them as diferent histological paterns [13,29]. Histological characteristics are atypia and anaplasia of cells from the mucous malpighian body with cellular loss of polarity and presence of some dyskeratotic cells, in both the basal and squamous layers [29].…”

“…The dermoscopic pigmentary structures of VIN may be similar to those visible in its male counterpart, including brown/brown–gray dots arranged in linear fashion, albeit they are usually more scattered compared to VIN, and blue–black structureless areas . The male lesions may also show glomeruloid vessels, but they are more diffuse and scattered than those visible in VIN …”
