Abstract

José Carlos Gomes Sardinha 5A hanseníase possui amplo espectro de manifestações clínicas relacionadas a três processos: dano celular e tecidual em função da multiplicação e disseminação dos bacilos; resposta imune do hospedeiro; e complicações do acometimento dos nervos periféri-cos.1 Nas formas clínicas em que o hospedeiro apresenta alta resistência, o bacilo raramente é demonstrado, as lesões são bem delimitadas e em pequeno número. Nas formas de baixa resistência, o bacilo é facilmente detectado, havendo grande número de lesões. Esse atributo é utilizado para determinação do esquema terapêutico adequado. Para fins operacionais, recomenda-se o esquema paucibacilar para pacientes com até cinco lesões, e, acima de cinco, o multibacilar. Essa regra, entretanto, nem sempre se aplica, pois é possível o doente apresentar poucas lesões e grande número de bacilos, ou vice-versa.
