The aim of this study is to describe key-elements of patient-centred care and its relationship with interprofessional collaborative practice in Primary Health Care, in the context of Unified Health System. Based on national and international review. The outcomes show that as professionals shift their focus towards patient centred care and patient's needs, their scope of view is broadened beyond the limits of their own professional activities. This shift is an enabler for changes in the current health care model towards comprehensive care and potentially impacting the quality of services.Keywords: Patient-centred care. Primary Health Care. COMUNICAÇÃO SAÚDE EDUCAÇÃO 2016; 20(59):905-16 IntroductionThe present study analyzes patient-centered care (PCC) in interprofessional collaborative practice (ICP) following two leads: the first is a consequence of the emphasis of the health policies and of the research centers that are devoted to ICP [1][2][3][4] in PCC, highlighting its essential role in interprofessional collaboration. The current literature is also in agreement in considering the PCC as a key element in teamwork and ICP 5 .The second lead for this study comes from the fact that in spite of the agreement in the literature seeing the PCC as a competency domain of the ICP 1,6 and the existence of successful empirical outcomes related to PCC 7,8 , there is a lack of a consensual definition for PCC, hampering its implementation 9 and hindering the dialogue between national and international literature. This dialogue is central to understand how international contributions on PCC may be comparable to the user-centered approaches present in Brazilian literature.The academic publications on the issue represent PCC as a central domain for interprofessional collaboration and a core element in teamwork and ICP 5 . A PCC-oriented team is an indicator for differentiating three increasing levels of collaboration 5 (potential collaboration, collaboration in development, active collaboration and in this classification the denomination of a group as a "team" is restricted to those presenting "active collaboration", that guide their actions with the patients and their health needs in focus. MethodsTo identify the Brazilian contributions on this subject, a review was performed in BDENF, LILACS and SciELO, using as descriptor the Portuguese equivalent of "patient centered care" e.g. "assistência centrada no paciente" without timeline delimiters. The search was performed from September 2013 to February 2014, using as criteria: Studies dealing with PCC as main subject and freely available in full-text versions, excluding thesis, dissertations and books. ResumoEste artigo apresenta hipóteses interpretativas sobre a ausência de regulamentação institucional do acesso -caracterizado por certo desvio e/ou subvalorização do mesmo -na atenção primária à saúde (APS) brasileira, especialmente na Estratégia Saúde da Família (ESF), no seu aspecto de prover acesso rápido ao cuidado clínico longitudinal aos adoecidos e ou demandantes. As hipóteses para esse problema foram contextualizadas em dois eixos: nas normativas do SUS e nas discussões presentes na área da saúde coletiva. No primeiro eixo apresenta-se uma discussão envolvendo a influência histórico-cultural norte-americana na saúde pública brasileira, a visão focalizada de APS como "cesta básica" de serviços, a priorização da promoção/prevenção nas diretrizes institucionais, o acolhimento (única diretriz estimuladora do acesso fácil na ESF/APS) e o dimensionamento da proporção usuários/equipes da ESF, o que fundamenta a defesa da expansão dessas equipes e o seu redimensionamento. No segundo eixo faz-se uma discussão crítica envolvendo o relativo isolamento brasileiro da experiência dos países desenvolvidos com APS forte, a influência da crítica das relações entre a biomedicina e o capitalismo, a ênfase na promoção e prevenção como objetivos prioritários de trabalho nas equipes da ESF/APS, o distanciamento do meio acadêmico das realidades dos serviços da ESF e a estratificação social que fomenta o uso subsidiado do sistema privado pelas camadas médias e elites brasileiras. Por fim, se propõe a revalorização do acesso fácil ao cuidado com longitudinalidade como fundamental para concretizar as quatro lógi-cas que devem convergir nas práticas da APS/ESF: ético-política, antropológica, epidemiológica e de determinação social da saúde-doença. Objective:To assess the health care process for women over 50 at a Family Health Unit based on the concept of programmatic vulnerability. Method: This study is inserted in the fi eld of health care assessments. The framework proposed by Donabedian was used to analyze 90.5% of the 790 records of women registered at the unit. Results: It was observed that none of the women that did not have a diagnosed pathology attended the recommended consultations or underwent the recommended tests. Of the total number of women with hypertension or diabetes, 20.7% were registered in the Hiperdia Programme and less than 1.0% had attended the consultations and undergone the necessary tests. Only 11.9% of the women had had a gynaecological examination, a clinical breast examination and a mammography the year before data collection. Conclusion: It is concluded that women over 50 are in a situation of programmatic vulnerability in terms of the indicators established in this study. Knowledge of this reality can help nurses provide care that is best suited for this group. Keywords: Primary health care. Women's health. Program evaluation. RESUMO Objetivo:Avaliar o processo de atenção prestada a mulheres, a partir dos 50 anos de idade, em Unidade de Saúde da Família, tomando-se como referência o conceito de vulnerabilidade programática. Método: Estudo inserido no campo da avaliação de programas de saúde; utilizou-se o referencial proposto por Donabediam, sendo analisados 90.5% dos 790 prontuários de mulheres matriculadas na Unidade. Resultados: Observou-se que nenhuma mulher sem patologia diagnosticada realizou consultas e exames preconizados. Do total de hipertensas e diabéticas, 20.7% estavam inscritas no Programa Hiperdia e menos de 1.0% tinha realizado as consultas e exames necessários. Apenas 11.9% das mulheres tinham realizado exame ginecológico, exame clínico das mamas e mamografi a, no ano que antecedeu a coleta de dados. Conclusão: Conclui-se que as mulheres com mais de 50 anos estão em situação de vulnerabilidade programática, relacionada aos indicadores defi nidos neste estudo. Logo, conhecer a realidade poderá resultar no atendimento de enfermagem mais adequado para esse grupo. Palavras-chave: Atenção primária à saúde. Saúde da mulher.
