Nursing process: from literature to practice. What are we actually doing?

Aim: to analyze whether the nursing process method is used at public and private centers in the health area Gipuzkoa (Basque Country) and, if yes, to analyze in the framework of which model and how it is used. Method: cross-sectional study, based on the analysis of the nursing records used at the 158 centers studied. Results: the nursing process is applied at 98% of the centers. It is applied at all public and 18 out of 21 private centers. Virginia Henderson's model is the most used to apply it, and most centers use nursing diagnoses, the NIC-NOC terminology and standardized care plans. Conclusion: the use of the nursing process is widespread in Gipuzkoa, with greater use at public than at private centers.Descriptors: Nursing Process; Nursing Diagnosis; Nursing Care; Models, Nursing. Uso del proceso de enfermería en los centros públicos y privados de un área de saludObjetivo: analizar si la metodología del proceso de enfermería se utiliza en los centros públicos y privados del área de salud de Gipuzkoa (País Vasco) y, en caso de utilizarse, analizar bajo qué modelo enfermero y de qué manera se utiliza. Método: estudio transversal, basado en el análisis de los registros de enfermería que utilizan los 158 centros estudiados. Resultados: el proceso de enfermería se aplica en el 98% de los centros estudiados. Se aplica en todos los centros públicos y en 18 de los 21 centros privados. El modelo de Virginia Henderson es el más utilizado para aplicarlo, y el uso de los diagnósticos enfermeros, de la terminología NOC-NIC y de los planes de cuidados estandarizados es mayoritario. Conclusión: se concluye que el uso del proceso de enfermería está extendido en Gipuzkoa, y su uso es mayor en los centros públicos que en los privados.

The visibility of hygiene self-care in nurse-to-nurse shift change reports

This study aimed to understand the visibility of hygiene care related to inpatients with compromised self-care through the nurse-to-nurse shift change report. A qualitative approach was used and data were collected through observation and semi-structured interviews. A total of 33 nurses from medical and surgical units were observed and eight of them were interviewed. The results indicate that the nurse-to-nurse shift change report is a cyclically recreated practice that results in group coherence in updating information and in the continuity of care, and keeps nurses united. Hygiene care activities are present in the shift change reports; they are frequent in the shift change from morning to afternoon and less frequent in the remaining. Information concerning these activities helps to determine the type of aid necessary to the continuity of hygiene and of many other care activities and the rhythm of the following shifts. La visibilidad del autocuidado higiene en el cambio de turno de los enfermerosEl objetivo fue comprender la visibilidad de los cuidados de higiene en la información del cambio de turno, inherentes a la persona internada con el autocuidado comprometido. Se recurrió al abordaje cualitativo, utilizando la observación y la entrevista semiestructurada.Fueron observados 33 enfermeros, 8 de los cuales fueron entrevistados, provenientes de un servicio de medicina y otro de cirugía. De los hallazgos, se puede destacar que el cambio de turno es una práctica recreada cíclicamente y apunta para el sentido de coherencia del grupo en la reactualización de la información, la continuidad de los cuidados y mantiene unidos a los enfermeros. Los cuidados de higiene muestran su presencia en los relatos del cambio de turno, con vínculo explícito en el turno de las 14h30min e implícito en los restantes cambios. La información sobre estos proporciona orientaciones para la continuación de dichos cuidados, así como de los restantes cuidados, actividades y ritmo del próximo turno.Descriptores: Investigación en Enfermaría; Autocuidado; Trabajo por Turnos.

Nursing Interventions in the Clinical Practice of an Intensive Care Unit

This cross-sectional study was carried out at a university hospital to describe the nursing interventions most frequently performed in the clinical practice of an intensive care unit, based on nursing care prescriptions, and to investigate their similarity to the Nursing Interventions Classification (NIC). The sample consisted of 991 hospitalizations of patients.Data were retrospectively collected from the computer database and analyzed through descriptive statistics and cross-mapping. A total of 57 different NIC interventions frequently used in the unit were identified; most of them in the complex (42%) and basic physiological (37%) domains, in the classes 'respiratory management' and 'self-care facilitation'. Similarity between the nursing care prescribed and nursing interventions/NIC was found in 97.2% of the cases. The conclusion is that the interventions/NIC used in the clinical practice of this intensive care unit reflects the level of complexity of nursing care, which is mainly directed at the regulation of the body's physical and homeostatic functioning. Los datos fueron recolectados retrospectivamente, en base informatizada, y analizados por la estadística descriptiva y diseño cruzado. Se identificó 57 diferentes intervenciones/NIC frecuentemente utilizadas en la unidad; siendo la mayoría en el dominio fisiológico complejo (42%) y fisiológico básico (37%), en las clases de control respiratorio y facilitación del autocuidado. En 97,2% de los casos se encontraron similares entre las prescripciones de enfermería de la unidad y las intervenciones/NIC. Se concluye que las intervenciones/NIC utilizadas en la práctica clínica de la unidad de terapia intensiva reflejan el nivel de complejidad del cuidado de enfermería en esta unidad, destinándose, principalmente, a regular el funcionamiento físico y homeostático del organismo.
