BackgroundAfter the early detection of cervical intraepithelial neoplasia (CIN), medical surveillance of the precancerous lesions is carried out to control risk factors to avoid the development of cervical cancer.ObjectiveTo explore the effects of medical surveillance on the personal and social lives of women undergoing CIN follow-up and treatment.MethodologyA generic qualitative study using a poststructuralist perspective of risk management was carried out in a gynecology clinic in a public hospital of the Galician Health Care System (Spain). Participants were selected through purposive sampling. The sample consisted of 21 women with a confirmed diagnosis of CIN. Semistructured interviews were recorded and transcribed, and a thematic analysis was carried out, including researcher triangulation to verify the results of the analysis.FindingsTwo main themes emerged from the participants’ experiences: CIN medical surveillance encounters and risk management strategies are shaped by the biomedical discourse, and the effects of “risk treatment” for patients include (a) profound changes expected of patients, (b) increased patient risk management, and (c) resistance to risk management. While doctors’ surveillance aimed to prevent the development of cervical cancer, women felt they were sick because they had to follow strict recommendations over an unspecified period of time and live with the possibility of a life-threatening disease. Clinical risk management resulted in the medicalization of women’s personal and social lives and produced great uncertainty.ConclusionsThis study is the first to conceptualize CIN medical surveillance as an illness experience for patients. It also problematizes the effects of preventative practices in women’s lives. Patients deal with great uncertainty, as CIN medical surveillance performed by gynecologists simultaneously trivializes the changes expected of patients and underestimates the effects of medical recommendations on patients’ personal wellbeing and social relations.

Objetivo: descrever o conhecimento de mulheres quilombolas sobre o câncer de colo uterino. Metodologia: estudo desenvolvido em comunidade quilombola, localizada no estado da Bahia. Os dados foram obtidos por meio de entrevista semiestruturada e oficina feminista, no período de julho a setembro de 2014. Resultados: as mulheres demonstraram dificuldade de entendimento sobre a doença e sua localização, sendo identificada como perigosa, feia e que mata; associada ao uso de pílulas anticoncepcionais, à não observância de cuidados tradicionais em relação ao parto, pós-parto e ao exercício livre da sexualidade pelas jovens. Conclusão: ficou evidente o desconhecimento das participantes sobre seu próprio corpo e sobre a etiopatogenia do câncer de colo uterino, mostrando a necessidade e a importância do estabelecimento de atividades educativas participativas.Descritores: Conhecimento; Neoplasias do colo do útero; Saúde da mulher; Grupo com ancestrais do continente africano; Enfermagem.

Aim To examine situations of vulnerability to human papillomavirus in the social representations of young women. Background Human papillomavirus is one of the most frequent sexually transmitted infections among women and constitutes a serious health problem among youth. This problem is embedded in psychosocial issues that promote situations of vulnerability and affect attitudes and behaviour in relation to prevention. Methods Qualitative study based on the theory of social representations, conducted among 64 female students in a high school in the city of Rio de Janeiro, Brazil. The study used personal semi‐structured interviews, with a lexical analysis by Alceste 2010 software. Findings The inefficacy of information on the human papillomavirus pointed out individual and social vulnerabilities, and showed relationships between the difficulty of access and the perception of low quality in public health services, which also promotes institutional vulnerability. Discussion The representation of vulnerability to human papillomavirus infection is associated, not only with individual behaviours related to sexuality and gender identity but also to social and institutional conditions involving insufficient information and low quality of and ineffective access to public health services. Conclusion Situations of vulnerability of young women to human papillomavirus infection influence ways of thinking and acting, when they generate feelings and behaviours that put them at risk. The social representations provided knowledge about how the participants appropriated consensual and reified universes in relation to vulnerability, influencing care and awareness of prevention. Implication for nursing and health policy Situations of vulnerability, whether individual, social or institutional, warrant the attention of nurses and should be prioritized as public health policy goals to reduce the number of cases of human papillomavirus infection among younger women.
