ResumoIntrodução: A medicina fetal tem se constituído, a partir da década de 1970, como área da medicina obstétrica que objetiva introduzir novas técnicas para detecção e cuidado de malformações embrionárias e fetais, por meio de um atendimento multiprofissional. Ao veicular a informação de que uma alteração na estrutura ou fisiologia do feto foi constatada, a equipe de saúde precisa estar preparada para responder e fornecer suporte às angústias do casal. Objetivo: Avaliar a formação do vínculo materno-fetal, bem como o enfrentamento de gestantes frente ao diagnóstico de malformação. Casuística e Métodos: Vinte gestantes atendidas pela Equipe de Medicina Fetal de um Hospital-Escola do interior do estado de São Paulo responderam a uma entrevista semiestruturada, à escala Hospitalar de Ansiedade e Depressão, à escala Modos de Enfrentamento de Problemas e à escala de Apego Materno-fetal. Os escores dos instrumentos foram correlacionados entre si, utilizando estatística descritiva e testes estatísticos não paramétricos. Resultados: Dezoito (90%) gestantes apresentaram apego máximo com seu feto, treze (65%) apresentaram indicadores de ansiedade e oito (40%), indicadores de depressão. A principal estratégia de enfrentamento utilizada por elas foi a busca de práticas religiosas, com sentimentos de esperança e fé. Conclusão: Estudos sobre essa temática possibilitam a discussão de estratégias de intervenção que propiciem o melhor enfrentamento das gestantes para adaptação à nova realidade do feto e uma relação materno-fetal mais saudável.Descritores: Gravidez; Adaptação Psicológica; Relação Materno-Fetal.
AbstractIntroduction: Fetal medicine has been established since the 1970s as the field of obstetric medicine. It aims to introduce new techniques for detection and care of embryonic and fetal malformations through a multidisciplinary care. When conveying information that a change in the structure or physiology of the fetus was found, the healthcare team must be prepared to respond and provide support to the couple's anguish. Objective: To evaluate the formation of the mother-child bond, as well as the coping strategies of pregnant women facing the diagnosis of malformation. Patients and Methods: Twenty pregnant women seen by the team of Fetal Medicine of a teaching hospital in the state of São Paulo were asked to answer a semi-structured interview, the hospital anxiety and depression scale, the ways of coping scale, and the maternal-fetal attachment scale. The instrument's scores have been correlated using both descriptive and nonparametric statistics. Results: Eighteen (90%) women have shown maximum attachment with their fetus, thirteen (65%) women had anxiety indicators and eight (40%) women have presented depression indicators. The main coping strategy used by them has been the search for religious practices with feelings of hope and faith.
