The shortage of physicians in remote and underprivileged areas poses an obstacle to universal access and quality of health care. Through the More Doctors Program (PMM), as of 2015, 18 thousand physicians had been incorporated into Brazil's Unified National Health System (SUS) to work in basic care, 79% of whom were Cubans. This article analyzed the comprehensiveness of practices by Cuban physicians in the PMM using a qualitative study in the city of Rio de Janeiro, Brazil, based on interviews with Cuban physicians (24) and a focus group with supervisors of the PMM (4). Comprehensiveness was analyzed in two dimensions: the community-oriented biopsychosocial approach to care and the range of activities in health promotion, prevention, and care. The work by Cuban physicians presents elements that are consistent with the comprehensiveness of practices in primary care, providing a wide range of care and services, in keeping with the health problems' complexity and the plurality of settings. These health workers show outstanding capacity for community interaction, a preventive focus, planning of activities, and positive interpersonal team relations. The study identified attitudes and techniques of solidarity, physician-patient bonding, and community accountability. Challenges were identified in the promotion of participatory practices with communities, the expansion of users' autonomy in clinical decisions, management of psychological problems, systematization of approach tools, and performance of invasive procedures. The study furnishes strong evidence that the PMM, in addition to medical consultations, provides comprehensive health care and contributes to strengthening basic care in Brazil.

Objetivo: Analisar as percepções dos médicos atuantes na Estratégia de Saúde da Família de um município de médio porte no estado de Minas Gerais sobre a formação e atuação neste nível de atenção. Métodos: Trata-se de uma pesquisa de abordagem qualitativa, exploratória e descritiva, que avaliou o universo de significados, crenças, valores e atitudes dos participantes em relação ao tema. Os dados foram coletados mediante entrevista semiestruturada. Resultados: Foram realizadas 12 entrevistas com médicos de 11 unidades de saúde. Os sujeitos entrevistados relatam que os conhecimentos referentes à Estratégia de Saúde da Família no currículo da graduação foram satisfatórios e contribuíram com sua formação e atuação profissional. No entanto, a escolha pelo trabalho ainda aparece como um caminho facilitador para entrada na residência médica, principalmente por meio de programas governamentais. Conclusões: Ainda que a pesquisa tenha abordado os relatos de uma pequena parcela de médicos, a análise evidencia a importância da educação médica na sensibilização e capacitação em medicina de família e comunidade, bem como a ainda necessária valorização da carreira no nível primário de atenção. Estas temáticas permanecem no bojo das discussões quando se trata da consolidação desta estratégia no país. Resumo Informes médicos acerca de la educación y trabajo en la Estrategia de Salud de la FamiliaObjective: To analyze the perception of physicians working in the Family Health Strategy of a medium-sized municipality in the state of Minas Gerais, about the training and work in this field of activity. Methods: This is a qualitative, exploratory and descriptive research, which addressed the universe of meanings, beliefs, values and attitudes of the participants towards the subject. Results: 12 interviews were conducted with physicians from 11 health units. The subjects interviewed report that the knowledge referring to the Family Health Strategy in their undergraduate curriculum was satisfactory and contributed to their professional training and performance. However, the choice of work still appears as a facilitating pathway to medical residency, mainly through government programs. Conclusions: Although the research has addressed the reports of a small number of physicians, the analysis highlights the importance of medical education in sensitizing and training for family and community medicine, as well as the still needed career enhancement in the primary care level. These topics remain in the heart of the discussions when it comes to the consolidation of this strategy in the country.

The More Doctors Program has been considered, in recent studies, the most important normative change of the State’s attributions in the organization of the education of human resources for the Brazilian National Health System (SUS). This article analyzes the most structuring axis of the program, education in health, by revising literature, and analyzing documents and databases in order to describe its model as a public policy and discuss which normative changes had an effect on the State’s action. The article identifies institutional changes in the Brazilian Ministry of Health and Ministry of Education, in the reorientation of medical education, in the expansion and redistribution of undergraduate and residency seats, and in the creation of public policy instruments to plan, regulate and assess the education of specialists. The paper is concluded by identifying interruptions in the program’s structuring actions and objectives, and goals and objectives that will probably not be met within the established deadlines.
