Patients undergo nutritional therapy so as to prevent malnutrition or worsening of the condition. When they are unable to receive oral nutrition, the most common form of physiological and nutritional therapy is through enteral nutrition (EN). The drug-nutrient interaction is described as occurring changes in the kinetics or dynamics of the drug or nutrient administered. The purpose of this study is to describe drug-nutrient interactions in prescriptions required for enteral nutrition in patients at the Intensive Care Unit of a General Hospital of the Federal District. A descriptive-exploratory study was undertaken with data collected from January 2011 to December 2012 at the Intensive Care Unit (ICU) of the North Wing Regional Hospital (Hospital Regional da Asa Norte -HRAN). The investigation for interactions in prescriptions was conducted using Micromedex ® software and Spearman correlation was used for the statistical analysis of variables. In the 101 prescriptions analyzed, 1190 drugs were found. The minimum amount of drugs found per patient was 7 and the maximum was 18, at an average of 12 drugs. Regarding enteral drug-nutrition interaction, the incidence was 15.94%, while drug-drug interaction occurred in 84.06% of all observed interactions. The most frequent interactions were moderate, in 44.57% of the incidences, followed by major interactions at 32.33%, and minor interactions at 13.16%, and lastly the contraindications in 9.93% of prescriptions analyzed. There was a high incidence of moderate degree drug interactions in patients under enteral nutrition therapy.

Objectives: This study aimed to analyze the profile of pharmaceutical clinical interventions performed concurrently with the medical prescription evaluation service. Methods: This is a cross-sectional, prospective and observational study conducted at the General Intensive Care Unit of a public hospital in Santa Catarina, Brazil, with 8 beds in full occupation. The study included 54 patients hospitalized from February to July 2017. Included in the study were medical prescriptions and pharmaceutical interventions recorded in the electronic medical record of patients of both genders of any race, origin, age and pathology. Medical prescriptions and pharmaceutical interventions that did not belong to the hospitalization unit studied and the study period were excluded from the study. Results: A total of 499 medical prescriptions belonging to 54 patients (48%), male patients (68%), hypertensive patients (27%), diabetic patients (22%) and polytrauma patients (13%) were analyzed. Of the prescriptions analyzed, 91.1% had a need for pharmaceutical intervention with a total of 64.2% acceptance. The main drug-related problems were drug interaction (40%), potential adverse effect (28%) and need for dose adjustment (13%). The classes of drugs that most needed intervention were analgesics (23%), cardiovascular (14%) and antimicrobials (13%). Conclusions: In assessing clinical pharmaceutical interventions as a benefit in addition to critical care in the Intensive Care Unit we emphasize the percentage of acceptance of pharmaceutical interventions (64%), mostly those involving increased risk for adverse effects and drug interactions, thus contributing to the prevention of complications.

<p>Estudo descritivo exploratório com objetivo de verificar a taxa de erros em prescrições médicas coletadas em uma drogaria privada. Foram analisadas 120 prescrições, coletadas durante 30 dias, de acordo com sete variáveis. No total foram identificados 426 erros. Os erros mais frequentes foram prescrições ilegíveis ou pouco legíveis 68,33%, identificação do prescritor incompleta ou omissa 10%, identificação do paciente incompleta ou omissa 42,5%, posologia incompleta ou omissa 40%, dose incompleta ou omissa 45%, presença de abreviaturas 84,17%, e 65% das prescrições não cumpriram de forma adequada com a legislação. Observa-se elevada taxa de erros nas prescrições analisadas, demonstrando a necessidade da avaliação do farmacêutico para a detecção destes erros, solicitação de correções e prevenção do risco de exposição do paciente aos problemas ocasionados por esses erros tais como reações adversas, interações medicamentosas, intoxicações, agravamento de doenças ou óbito. Ademais, é necessário conscientização para o cumprimento das boas práticas de prescrição e sugere-se a realização de estudos para elaborar estratégias que minimizem a ocorrência de erros de prescrição.</p>
