Health-related quality of life among the elderly with heart failure: a generic measurement

CONTEXT AND ObjECTivE: Health-related quality-of-life (HRQoL) instruments have been greatly used among patients with heart failure (HF), although few of them are specific for elderly people. Among the generic instruments, the Medical Study 36-item Short-Form Health Survey (SF-36) is widely used. The aim here was to evaluate HRQoL among elderly individuals with HF through this generic instrument.DESiGN AND SETTiNG: Cross-sectional study at two university hospitals in São Paulo, Brazil.METhODS: 170 elderly people with HF who were being followed up as outpatients were interviewed. To evaluate HRQoL, SF-36 was used. RESulTS:The sample was composed of subjects with a mean age of 67.5 (± 6.2) years, with a diagnosis of HF for 65.9 (± 42.4) months, in functional class I (38.8%; 66) or II (42.9%; 73) and with reduced left ventricular ejection fraction (LVEF) (51.2%). The mental and social HRQoL domains did not seem to be compromised, since they presented high scores. Patients with HF typically had impaired physical capacity, which may explain the lower scores in the physical domain. Cronbach's alpha coefficients were greater than 0.77 for all dimensions, except for general health status. RESulTADOS: A amostra foi composta por sujeitos com média de idade de 67,5 (± 6.2) anos, com tempo médio de IC de 65,9 (± 42.4) meses, em classe funcional I (38,8%, 66) ou II (42,9%, 73) e fração de ejeção do ventrículo esquerdo (FEVE) rebaixada (51,2%). Os domínios da QVRS que avaliam os aspectos mental e social não parecem estar comprometidos, uma vez que apresentaram altas pontuações. Pacientes com insuficiência cardíaca geralmente têm a capacidade física prejudicada, o que pode justificar a pontuação mais baixa no domínio físico. Os coeficientes alfa de Cronbach foram superiores a 0,77 para todas as dimensões, exceto Estado Geral de Saúde.CONCluSãO: A medida da QVRS pelo SF-36 apresentou alto nível de confiabilidade quando aplicado em idosos brasileiros com IC. Essa população apresentou escores mais baixos nas dimensões capacidade funcional e aspectos físicos, fornecendo subsídios para estudos de intervenções que contribuam para a otimização da QVRS desse grupo.

Objective:To assess the quality of life in elderly patients with heart failure. Methods: Cross-sectional study conducted in two university hospitals. Interviews were conducted with 170 elderly patients with heart failure in an outpatient setting. To assess the quality of life, the Minnesota Living With Heart Failure Questionnaire was used. Results: The results showed a minor impact of illness on the emotional dimension of quality of life. However, indicators related to the physical dimension, including fatigue and breathlessness, showed a greater impact on quality of life for elderly patients. The internal consistency of the instrument-specific to quality of life was high, indicating satisfactory reliability (α> 0.80). Conclusion: This study enabled the assessment of quality of life of elderly patients living with heart failure, demonstrating that the physical dimension was the most compromised. It is essential to implement strategies to improve physical functioning in these patients. É essencial a implementação de estratégias que melhorem a função física desses pacientes. Aims Home‐based interventions for heart failure (HF) patients might be particularly effective in middle‐income countries, where social, cultural, and economic constraints limit the effectiveness of HF treatment outside the hospital environment. Methods and results HELEN‐II was a randomized clinical trial conducted in Brazil designed to evaluate the clinical efficacy of a nurse‐based strategy, started after discharge following an acute decompensated HF (ADHF) admission. HELEN‐II compares the efficacy of home visits and telephone reinforcement (n = 123) with that of the conventional strategy, which is based on medical follow‐up (n = 129). The primary outcome was a composite endpoint of a first visit to the emergency department (≤24 h), a hospital readmission (>24 h), or all‐cause death, assessed during the first 6 months of follow‐up. Most enrolled subjects were middle‐aged (62 ± 13 years) males (63%) in NYHA functional class II–III (84%) with severe LV dysfunction (mean LVEF 29.6 ± 9%). The primary composite endpoint was decreased by 27% in the interventional group (relative risk 0.73; 95% confidence interval 0.54–0.99; P = 0.049). At the end of follow‐up, the rate of use of the standard‐of‐care HF medications was similar in both groups, except for the higher use of furosemide in the interventional group. Also, HF knowledge and self‐care were significantly increased in the interventional group. Conclusions A post‐discharge, nurse‐led management strategy significantly decreases the morbidity of ADHF patients in the public health system of a developing middle‐income country.
