Background: In Brazil, primary health care assumes the challenge of organizing and articulating health care networks, based on the performance of multidisciplinary teams linked to basic health units. Given that there are limited studies on the organization of the work process of primary health care teams in Brazilian municipalities that consider the rural-urban typology proposed by the Brazilian Institute of Geography and Statistics, this study aims to analyze the work process of primary health care teams in Brazilian municipalities.Methods: Quantitative evaluation was conducted using secondary data from the third cycle of the National Program for Improving Access and Quality of Primary Care (PMAQ-AB) and the Rural/Urban Classification of the Brazilian Institute of Geography and Statistics. Overall, 37,350 teams that participated in all levels of the PMAQ-AB external evaluation were included. The descriptive analysis considered three axes: 1) territorialization, 2) action planning, and 3) monitoring and self-evaluation. Multiple correspondence analyses were used to verify the relationships between the variables and municipal classifications.Results: Teams from adjacent rural municipalities had the lowest percentage of uncovered population in the territory (21.0%), while those from urban municipalities had the highest percentage with reference population >3,500 people/team (43.0%). It was found that 5,446 (15.1%) teams did not carry out planning, and the highest percentage of teams that executed planning was in the urban strata (85.7%). Monitoring performance was observed in 87.9% of the teams, with approximation of the values among the municipal strata with similar characteristics of location; self-evaluation showed the highest percentage in the adjacent intermediate stratum (90.4%). It was possible to identify three groups of teams with distinct characteristics of territorialization and two groups in relation to planning, monitoring, and self-evaluation.Conclusion: The results suggest important differences in the work process of primary health care teams, which vary according to the characteristics of the municipalities in which they are located.

Introdução É notória a relação entre medidas de saúde e variáveis sócio-econômicas, estudada segundo diferentes abordagens teóricas. Do ponto de vista econômico, por exemplo, isso se deve à importância da saúde como um determinante fundamental da capacidade produtiva das pessoas 1,2 . Ter boa saúde significa estar mais disposto a exercer atividades dentro e fora do mercado de trabalho. No campo sociológico, os estudos focam a interação entre condições de saúde e fatores socioculturais e biossociais, como status marital, religião, posição na família, raça, gênero, entre outros 3,4 . Esses fatores refletem a posição do indivíduo na sociedade e condicionam sua forma de viver e interagir, com conseqüências para a saúde. De forma análoga, a epidemiologia social procura analisar como os determinantes sócio-econômicos e comportamentais influenciam a distribuição de doenças no âmbito populacional 5,6 .A despeito do arcabouço teórico utilizado, é amplamente difundida a importância dos determinantes sociais sobre as condições de saúde de indivíduos e populações 7,8 . Determinantes esses dados por fatores econômicos, educacionais, sociais, culturais, raciais, psicológicos e comportamentais, que influenciam a ocorrên-cia de doenças na população e sua exposição a fatores de risco 9 . Can credit help small family farming? Evidence from Brazil

Purpose The purpose of this paper is to evaluate the impact of a Brazilian rural credit program, The National Program for Strengthening Family Farming (PRONAF), on small family farming production. Design/methodology/approach The method is based on a quasi-experimental approach (propensity score matching) applied to 4.1m family farmers in Brazil. Findings Results show that farmers accessing PRONAF tended to be positively selected in terms of several observable characteristics, such as land size and agricultural practices. Moreover, PRONAF had positive and differentiated impacts on agricultural production. The impact was larger in the poorest region when compared to the regions characterized by intensive and commercial farming. Research limitations/implications The rural credit information was restricted to one crop year, making impossible to analyze the mid- and long-term impacts of the credit program on agricultural production. Practical implications The study provides some practical implications for policies of rural development. First, rural credit does matter for agricultural production of small family farmers. Nonetheless, since credit programs are large subsidized by the rest of the population, further studies are still needed the aggregate costs and benefits of these schemes. Results also revealed that PRONAF may have contributed to reduce regional inequalities, since the impact was larger in the poorest NE region. Originality/value This study provides a comprehensive analysis of how rural credit has impacted small-farm agricultural production, using large and representative data – the whole population of Brazilian family farmers.
