Radiation therapy is one of the most common treatment strategies for thorax malignancies. One of the considerable limitations of this therapy is its toxicity to normal tissue. The lung is the major dose-limiting organ for radiotherapy. That is because ionizing radiation produces reactive oxygen species that induce lesions, and not only is tumor tissue damaged, but overwhelming inflammatory lung damage can occur in the alveolar epithelium and capillary endothelium. This damage may result in radiation-induced pneumonitis and/or fibrosis. While describing the lung response to irradiation generally, the main focus of this review is on cytokines and their roles and functions within the individual stages. We discuss the relationship between radiation and cytokines and their direct and indirect effects on the formation and development of radiation injuries. Although this topic has been intensively studied and discussed for years, we still do not completely understand the roles of cytokines. Experimental data on cytokine involvement are fragmented across a large number of experimental studies; hence, the need for this review of the current knowledge. Cytokines are considered not only as molecular factors involved in the signaling network in pathological processes, but also for their diagnostic potential. A concentrated effort has been made to identify the significant immune system proteins showing positive correlation between serum levels and tissue damages. Elucidating the correlations between the extent and nature of radiation-induced pulmonary injuries and the levels of one or more key cytokines that initiate and control those damages may improve the efficacy of radiotherapy in cancer treatment and ultimately the well-being of patients.

Background and aims: One of the main potential problems of radiotherapy (RT) in breast cancer is the increase in treatment-related inflammation. This study aimed to investigate the effect of eight weeks of moderate-intensity aerobic training (AT) on serum levels of tumor necrosis factor α (TNF-α) and interleukin-6 (IL-6) in rats undergoing RT. Materials and Methods: In this experimental study, 32 male rats were randomly divided into four groups of eight members including (1) healthy control (C), (2) AT, (3) RT, and (4) AT + RT groups. Rats were first anesthetized with ketamine-xylazine solution (K, 60-90 kg/mg; Z, 6-10 kg/mg) and then located on a Plexiglas plate with a thickness of 1 cm. Photon beam RT was performed using X-rays with a dose of 11 Gy from an Elekta compact linear accelerator (Elekta Compact 6-MV China). AT program was performed for eight weeks, five days a week, and one session a day for 60 minutes (70-75% of VO2max). Afterwards, one-way ANOVA was run to examine the research variables. Results: According to the results, TNF-α was significantly higher in the RT group compared to the C group (P=0.003), whereas IL-6 levels were significantly lower in the AT + RT group compared to the RT group (P=0.001) and the C group (P=0.027). Further, the levels of this cytokine were significantly lower in the AT group compared to the RT group (P=0.006) and similarly in the RT group compared to the C group (P=0.03). Conclusion: RT led to an increase in pro-inflammatory cytokines; however, performing AT could only significantly reduce the IL-6 levels.

The Potential of Physical Exercise to Mitigate Radiation Damage—A Systematic Review

There is a need to investigate new countermeasures against the detrimental effects of ionizing radiation as deep space exploration missions are on the horizon.Objective: In this systematic review, the effects of physical exercise upon ionizing radiation-induced damage were evaluated.Methods: Systematic searches were performed in Medline, Embase, Cochrane library, and the databases from space agencies. Of 2,798 publications that were screened, 22 studies contained relevant data that were further extracted and analyzed. Risk of bias of included studies was assessed. Due to the high level of heterogeneity, meta-analysis was not performed. Five outcome groups were assessed by calculating Hedges' g effect sizes and visualized using effect size plots.Results: Exercise decreased radiation-induced DNA damage, oxidative stress, and inflammation, while increasing antioxidant activity. Although the results were highly heterogeneous, there was evidence for a beneficial effect of exercise in cellular, clinical, and functional outcomes.Conclusions: Out of 72 outcomes, 68 showed a beneficial effect of physical training when exposed to ionizing radiation. As the first study to investigate a potential protective mechanism of physical exercise against radiation effects in a systematic review, the current findings may help inform medical capabilities of human spaceflight and may also be relevant for terrestrial clinical care such as radiation oncology.
