Abstract: PURPOSE: To compare the hemodynamic repercussions following a toxic dose of levobupivacaine and bupivacaine intravascularly injected in swines. Methods: Large White pigs were anesthetized with thiopental, tracheal intubation was performed and mechanical ventilation was instituted. Hemodynamic variables were recorded with invasive pressure monitoring and pulmonary artery catheterization (Swan-Ganz catheter). After a 30-minute resting period, the animals were randomly divided into two groups in a double-blinded … Show more

“…Compared with humans we applied huge doses of bupivacaine in this pig study because in pigs very high bupivacaine doses are needed to provoke severe hemodynamic disturbances. Udelsmann and colleagues [17] injected 4 mg/kg bupivacaine intravenously in 40 healthy pigs. The subjects tolerated the drug and recovered spontaneously to nearly baseline cardiovascular values.…”

“…Eine i.v.-Bupivacain-Bolusinjektion von 4 mg/kgKG wurde von den Tieren mit nahezu vollständiger spontaner Erholung der kardiovaskulären Parameter toleriert [26]. In einer weiteren Studie wurde Bupivacain kontinuierlich mit 1 mg/kgKG/min infundiert [19]; hierbei trat bis zu einer Dosis von 6 mg/kgKG kein Herz-KreislaufStillstand auf.…”

“…Bupivacaine in spinal anesthesia had been reported to induce ECG changes 9,10 . However, levobupivacaine has been shown to be less cardiotoxic 11 . On the other hand, high sympathetic blockage and hemodynamic changes that occur due to regional anesthesia and the inotropic agents used may cause proarrhythmic effects 10 .…”
