Abstract: Purpose:To report the initial experience in our country with a new technique for twin to twin transfusion syndrome (TTTS) treatment, using laser to coagulate the placental vessels responsible for the twin transfusion during gestation. Methods: Prospective study of eight cases diagnosed with TTTS evaluated in our service from january 2001 to june 2005. Through percutaneous introduction of a 2,0mm diameter fetoscope in the uterine cavity, placental surface vessels where directly visualized. Those identified as r… Show more

“…Conventional procedures use surgical tools for both diagnosis (e.g., endoscope, CCD camera) and treatment (laser device), which requires two incisions. The use of endoscopes with a small channel for inserting standard laser fibers [7] is hampered by difficulties with accurately aiming laser energy at a target from a two-dimensional endoscopic view, particularly when the endoscope and laser fibers are misaligned.…”

“…Therefore, repair of fetal defects has gradually migrated to other techniques, such as endoscopic fetal surgery or fetoscopy. The fetoscopic approach was initially established for treating feto-fetal transfusion syndrome (3,4) and, recently, for the repair of congenital diaphragmatic hernia (5) .…”

“…Fetoscopy is performed through a single percutaneous entry until the uterine cavity is reached (3)(4)(5) . One of the main complications of this type of procedure is the premature rupture of membranes, and this risk seems to be directly related to the diameter of the instrument used.…”

“…[8][9][10] Também já está estabelecida a coagulação dos vasos placentários a laser nos casos de gestações gemelares acometidas pela síndrome de transfusão feto-fetal. 11,12 Entre as anomalias do sistema nervoso central (SNC) a MMC é a mais freqüente e apresenta incidência de 3 por mil nascidos vivos. 13 Observa-se alto índice de seqüelas mesmo após a correção neonatal.…”
