BackgroundLeprosy continues to be a public health problem in many countries. Difficulties faced by health services include late diagnosis, under-reporting of new cases, adequate monitoring of disabilities and treatment. Furthermore, systematic follow-up after completion of treatment is important, when new disabilities may occur, or existing disabilities may get worse. The objective of the present study was to determine the prevalence of leprosy-associated grade 2 disabilities (G2D) after completion of multidrug therapy (MDT) and to identify factors associated with G2D.MethodsWe performed a cross-sectional study of 222 leprosy cases registered in Vitória da Conquista, Bahia state, Brazil from 2001–2014. We performed a clinical examination of the study participants and collected socio-economic and clinical information by interview. We identified factors associated with grade 2 disability (G2D) using logis tic regression.ResultsIn total, 38 (17.1%) participants were diagnosed with G2D, and 106 (47.7%) with grade 1 disabilities (G1D). The following independent factors were significantly associated with G2D: occurrence of leprosy reaction (adjusted OR = 2.5; 95%CI = 1.09–5.77), thickening and/or tenderness of one or more nerve trunks (adjusted OR = 3.0; CI = 1.13–8.01) and unemployment (adjusted OR = 7.17; CI = 2.44–21.07).ConclusionsThis study shows that physical disabilities remain after completion of MDT and frequently occur in an endemic area in Brazil. Finding new ways to reduce the burden of disability are urgently needed, and may include systematic follow-up of patients after treatment completion combined with evidence-based preventative measures.

Protein profile of leprosy patients with plantar ulcers from the Eastern Amazon region

BackgroundStudies investigating the nutritional status of patients with leprosy and plantar ulcers are sparse. Therefore, the objective of this study was to describe the protein profile of leprosy patients with plantar ulcers from the Eastern Amazon region.MethodsA case record form was created for 75 patients with leprosy (31 with plantar ulcers and 44 without plantar ulcers) with the following data: sociodemographic characteristics, clinical form of leprosy, presence or absence of plantar ulcers, and nutritional assessment using anthropometry consisting of the measurement of body mass index, arm circumference, arm muscle circumference, and triceps skinfold. Levels of blood albumin, transferrin, and C-reactive protein (CRP) were also measured. Data regarding protein intake were obtained using a Food Frequency Questionnaire.ResultsPlantar ulcers occurred more frequently in male patients (67.7%), patients aged 40–49 years (mean ± SD: 47.3 ± 8.0 years), and patients receiving 300 or 600 USD (71.0%). The mean weight and height of patients were 71.6 ± 11.4 kg and 1.62 ± 0.1 m, respectively. High levels of CRP were detected in 51.6% of leprosy patients with plantar ulcers and only 9.1% of patients without plantar ulcers (P < 0.001). Nutritional depletion of transferrin was observed in 14.3% of patients with paucibacillary leprosy and 44.3% of patients with multibacillary leprosy (P = 0.0447). Most patients had normal levels of serum albumin (74.2% with plantar ulcers and 77.3% without plantar ulcers).ConclusionsMost leprosy patients with plantar ulcers have normal levels of serum albumin and transferrin and high CRP levels, which indicates the presence of an inflammatory process. Our findings suggest the need to monitor patients with leprosy to prevent the occurrence of plantar ulcers and to provide adequate treatment for patients with existing plantar ulcers.Electronic supplementary materialThe online version of this article (doi:10.1186/s40249-017-0318-y) contains supplementary material, which is available to authorized users.

RESUMO: O objetivo da pesquisa foi analisar o perfil epidemiológico da hanseníase no município de Maricá, Rio de Janeiro. Estudo epidemiológico retrospectivo, com abordagem quantitativa. Utilizou dados do Sistema de Informação de Agravos de Notificação, de pacientes com hanseníase, no período entre 2000-2013. Incluiu as variáveis: número de casos notificados; idade; sexo; formas clínicas; classificação operacional; o grau de incapacidade dos acometidos e saída do sistema. Totalizaram 191 casos de hanseníase, sendo a maioria constituída de homens-54%, maiores de 15 anos-96%, multibacilares-63% e alta por cura como principal tipo de saída do sistema-87,5%. A taxa de detecção anual permaneceu média entre os maiores de 15 anos. Todos os examinados apresentaram algum grau de incapacidade física, durante o diagnóstico. Conclui-se que o conhecimento epidemiológico da doença na cidade implicará mais suporte para o controle de saúde pública local. Palavras-Chave: Hanseníase; epidemiologia; enfermagem; saúde pública. ABSTRACT: The study aimed at analyzing epidemiological profile of leprosy in the city of Maricá, Rio de Janeiro, Brazil, from 2000 to 2013. This is a retrospective epidemiological study with quantitative approach. Data collected of all leprosy cases from Notifiable Diseases Information System Database through the notification of patients with leprosy. The variables included were: new cases diagnosed within the 2000-2013 span; age; clinic forms; operational classification; degree of physical disability and system output. 191 patients were reported: 54% males, 96% are older than 15 years old, 34% were in the virchowian leprosy, tuberculoid (30%), dimorphic (19%) and indeterminate (13%), 63% multibacillary forms, 87.5% had had discharges after cure as main type of system output. Annual detection rate remained medium for those over 15. All those examined showed physical disability to a certain extent at diagnosis time. We concluded that the epidemiological knowledge of leprosy in the city must require stronger support for disease control.
