ResumoOBJETIVO: Descrever as características maternas e das crianças, bem como avaliar os fatores associados à interrupção do aleitamento materno exclusivo. MÉTODOS: Estudo transversal que incluiu 667 crianças de seis meses que compareceram na Campanha de Multivacinação em Uberlândia, 2008. No delineamento amostral, foram sorteadas as unidades de vacinação e posteriormente as crianças em cada unidade, sistematicamente. Um instrumento semiestruturado foi utilizado para coleta dos dados, formulado com questões sobre alimentação da criança e características sociodemográficas da mãe. Foi utilizado risco relativo e teste do χ 2 para análise dos dados, aceitando como nível crítico p<0,05, com intervalo de confiança de 95%. RESULTADOS: A prevalência do aleitamento materno para os menores de 120 e 180 dias foi de 89,5 e 85%, respectivamente; e na modalidade de aleitamento materno exclusivo, 50,6 e 39,7% para menores de 120 e 180 dias, respectivamente. Os fatores mais associados ao abandono do aleitamento materno exclusivo em menores de seis meses foram o trabalho materno fora de casa (OR=2,7; IC95%=1,7-4,2) e o uso de chupetas (OR=4,2; IC95%=2,8-6,3). O fato de a mãe ser multípara (OR=0,5; IC95%=0,4 -0,81) e recorrer ao atendimento puerperal na rede pública (OR=0,5; IC95%=0,3-0,7) representaram fatores de proteção contra a prática do desmame precoce. CONCLUSÃO: As prevalências do aleitamento materno e aleitamento materno exclusivo no município de Uberlândia estão entre as maiores do país. Os fatores mais frequentemente associados à prática de desmame precoce foram trabalho materno fora de casa, oferta de bicos ou chupetas às crianças, atendimento puerperal efetuado no serviço privado e primiparidade. Abstract PURPOSE:To describe the characteristics of mothers and children and to evaluate the factors associated with discontinuation of exclusive breastfeeding. METHODS: A cross-sectional study on infants under six months of age who attended the vaccination campaign in 2008. In the sample design, vaccination units were selected by drawing lots and infants were similarly selected later at each unit systematically. A semi-structured instrument was used for data collection, containing questions about children's nutrition and socio-demographic characteristics. We used Odds Ratio and the χ 2 test for data analysis, accepting as the critical level p<0.05. RESULTS: The prevalence of breastfeeding for infants under 120 and 180 days of age was 89.5 and 85% respectively, and exclusive breastfeeding was 50.6 and 39.7% for infants less than 120 and 180 days of age, respectively. The factors most related to the abandonment of exclusive breastfeeding in infants under six months were maternal employment outside the home (OR=2.73; 95%CI=1.74-4.29) and use of pacifiers (OR=4.26; 95%CI=2.85-6.38). The mother being multiparous (OR=0.57; 95%CI=0.40-0.81) and receiving postpartum care in the public health care network (OR=0.55; 95%CI=0.39-0.79) represented protective factors against the practice of early cessation of breastfeeding. ResumoIntrodução: A duração média do aleitamento materno ainda é insuficiente e apresenta grandes variações em razão do local e das características da população estudada. Objetivo: Analisar os fatores associados ao desmame em crianças sul brasileiras de comunidades de baixa renda. Material e método: Estudo transversal realizado através de entrevista envolvendo 124 mães de crianças com até 12 meses de idade, atendidas pela Pastoral da Criança, em Almirante Tamandaré, Paraná, Brasil. O risco ao desmame foi considerado como variável resposta e os dados do questionário, variáveis preditoras. Os dados foram analisados através do método de Kaplan-Meier, modelo de regressão de Cox e correlação de Spearman (α = 0,05). Resultado: A tábua de vida atuarial revelou uma probabilidade condicional de desmame maior aos 12 meses de idade, seguido do primeiro e quarto meses. A análise multivariada mostrou que o contato com a mamadeira antes do sexto mês de vida [HR=17,16 (2,86)] e o contato com a chupeta antes do sexto mês de vida [HR=3,48 (1,90-6,38)] foram variáveis de risco associadas ao desmame. Entre as crianças desmamadas, por ocasião da entrevista, a duração da amamentação apresentou correlação positiva com a idade do primeiro contato com açúcar (r s =+0,419; p=0,001) e negativa com a duração do uso de chupeta (r s =-0,300; p=0,017). Conclusão: O uso da mamadeira e da chupeta estiveram relacionados a uma menor duração do aleitamento materno nestas comunidades.Descritores: Aleitamento materno; desmame; fatores de risco. AbstractIntroduction: The average duration of breastfeeding is still insufficient and differ widely taking in account the location and specific characteristic of local people involved in these studies. Aim: This research aimed at studying factors associated to weaning among South Brazilian children living in a low-income household. Material and method: Cross-sectional study was carried out enrolling 124 mothers of 1 to 12-month-old children, who were interviewed at "Pastoral da Criança" (non-governmental organization) in Almirante Tamandaré, Paraná, Brazil. The risk of weaning considered was the dependent variable and the information collected from the questionnaires, independent variables. The method used to collect information in this study was Kaplan-Meier method and Cox regression model and Spearman's rank correlation (α = 0.05). Result: Actuarial life table presented a higher weaning conditional probability at 12 months of age, followed by first and fourth months of age. Validação para língua portuguesa do instrumento de avaliação do aleitamento materno LATCH

Objective: To translate into Portuguese and validate the LATCH breastfeeding assessment scale. Methods: This was a methodological study through translation and validation of the instrument. The following steps were followed: translation, back-translation, committee of judges, pre-test and validation. The validation process includes the analysis of 160 women in breastfeeding in the first postpartum days during hospitalization. Results:The results indicate that the LATCH instrument translated into Portuguese can be used by nurses for breastfeeding assessment, enabling the early detection of possible problems presented by the mother-baby binomial during breastfeeding. Conclusion: The study fulfilled the objective of adapting the LATCH breastfeeding assessment instrument to the Portuguese language and verifying its psychometric properties. The LATCH instrument validity measures what it was designed to do and is applicable in clinical practice. ResumoObjetivo: Traduzir para a língua portuguesa e validar a escala de avaliação da amamentação LATCH. Métodos: Tratou-se de um estudo do tipo metodológico, por meio da tradução e validação de instrumento. Seguiu-se as etapas estabelecidas: tradução, back-translation, comitê de juízes, pré-teste e validação. O processo de validação inclui a análise de 160 mamadas de mulheres nos primeiros dias pós-parto, durante a internação hospitalar. Resultados: Os resultados obtidos indicam que o instrumento LATCH traduzido para o português pode ser utilizado na avaliação do aleitamento materno pelo profissional enfermeiro, possibilitando a detecção precoce de possíveis problemas apresentados pelo binômio mãe-bebê durante a amamentação. Conclusão: O estudo cumpriu o objetivo de adaptar para a língua portuguesa e verificar as propriedades psicométricas do instrumento de avaliação de aleitamento materno LATCH. A validade do instrumento LATCH mede o que foi concebido para medir, sendo ele aplicável na prática clínica.
