Objective To evaluate prevalence trends of hypospadias in South-America it is essential to perform multicenter and multinational studies with the same methodology. Herein we present systematic data as part of an international multicenter initiative evaluating congenital malformations in South America over a 24-year period.Materials and Methods A nested case-control study was conducted using the Latin American Collaborative Study of Congenital Malformations (ECLAMC), between January 1989 and December 2012. Cases were stratified as isolated (IH) and non-isolated hypospadias (NIH). Global prevalence was calculated and discriminated by country. Associations between birth weight and gestational age, and NIH distribution by associated abnormality and severity of hypospadias, were analyzed.Results A total of 159 hospitals from six countries participated, reporting surveillance on 4.020.384 newborns. A total of 4.537 hypospadias cases were detected, with a global prevalence of 11.3/10.000 newborns. Trend analyses showed in Chile, Brazil and Uruguay a statistically significant increase in prevalence. Analysis of severity and associated anomalies did not to find an association for distal cases, but did for proximal (RR=1.64 [95% CI=1.33-2.03]).Conclusion This is one of only a few Latin American multicenter studies reporting on the epidemiology of hypospadias in South America in the last two decades. Our data adds to evidence suggesting an increase in some countries in the region at different times. There were also variations in prevalence according to severity. This study adds to literature describing associated anomalies at a hospital-based level.

Resumo O uso de mistura de agrotóxicos constitui prática relativamente comum na agricultura. A saúde humana pode ser afetada pelos agrotóxicos diretamente ou através do contato com produtos e/ou ambientes por estes contaminados-e, indiretamente, através da contaminação da biota de áreas próximas a plantações. A exposição concomitante a mais de um agrotóxico, como os herbicida sulfentrazona e atrazina ou os fungicida triazois e estrobilurinas; pode ocasionar toxicidade reprodutiva e/ou no desenvolvimento dos organismos expostos por possuírem potencial de interferirem no sistema endócrino como verificado experimentalmente. A exposição a vários agrotóxicos concomitantemente pode também afetar a saúde de trabalhadores rurais. Desta forma, é importante determinar as concentrações de misturas de agrotóxicos que causam efeitos prejudiciais em espécies não-alvo. Este quadro mostra a necessidade de realizar estudos experimentais relacionados à exposição conjunta de agrotóxicos e aprimorar a sua metodologia. Tais metodologias poderiam predizer as condições sob as quais as interações têm probabilidade de ocorrer. A compreensão do conceito de toxicidade de misturas e o desenvolvimento da capacidade para calcular quantitativamente a toxicidade das mesmas podem ser ferramentas úteis para se determinar as vantagens e desvantagens do uso de misturas. Há, portanto, necessidade de construir uma agricultura que considere os aspectos sociais, econômicos e ambientais.

Since the 2000s, both the production of genetically modified (GM) soybeans and the cases of agrochemical exposure have grown exponentially in Argentina. Drawing on ethnographic research, I analyze how peasant social movements understand the socioenvironmental problems caused by the expansion of GM soybeans. I argue that at national, provincial, and local scales, the institutional recognition of peasant social movements and the performative actions of authorities discourage contentious collective action through subtle yet powerful mechanisms. The article contributes to social movement research and to the literature on peasant resistance by analyzing the cultural dynamics that constrain contention and shape processes of peasant collaboration, which are arguably as important as peasant resistance, although much less studied.
