This study analyzes access to medicines among displaced and non-displaced populations in urban areas in Bucaramanga, Colombia. A household survey was carried out to study access to medicines for self-reported and medically diagnosed health conditions. Multiple Poisson regression with robust variance was used to determine factors associated with access to medicines. Two thousand and sixty individuals from 514 families participated. Only 29.1% (95%CI: 22.04-37.08) of the individuals in the sample with prescriptions and 44.3% (95%CI: 40.42-48.25) with self-reported needs for pharmacotherapy were taking medicines. Greater access was associated with the perceived severity of the illness, higher income, having a health center nearby and not perceiving barriers in accessing services. Social security affiliation and being displaced were not related. Social security coverage alone does not have an effect on access to medicines because it does not include essential medicines that correspond to the health needs of this population. Resolving administrative and geographical barriers is likely to improve access to medicines.

RESUMENObjetivo Contribuir al conocimiento sobre el acceso a los servicios en Colombia tras la reforma del sistema de salud, exponiendo los principales resultados y vacíos en las investigaciones. Métodos Se realizó una revisión sistemática de la bibliografía, a través de la búsqueda exhaustiva y análisis de artículos originales publicados entre 1994 y 2009. Se incluyeron 27 investigaciones cuantitativas y cualitativas que cumplían los criterios de selección. El análisis se enmarcó en los modelos teóricos de Aday y Andersen y Gold, que diferencian entre acceso potencial y realizado y consideran las características de la población, proveedores y aseguradoras que influyen en la utilización. Resultados Los análisis explicativos de la utilización de los servicios de salud a partir de modelos de determinantes resultan escasos y parciales (limitados a áreas geográficas, patologías o colectivos específicos). Pocos estudios profundizan en factores de contexto -políticas y características de proveedores y aseguradoras-o en la perspectiva de los actores sobre los factores que influyen en el acceso. Los estudios no parecen indicar un aumento del acceso realizado -salvo en el régimen subsidiado-y, en cambio, señalan la existencia de importantes barreras relacionadas con factores poblacionales (aseguramiento, renta y educación) y características de los servicios (accesibilidad geográfica, organizativas y calidad). Conclusiones La revisión muestra limitaciones importantes en el análisis del acceso en Colombia que indican la necesidad de reorientar la evaluación hacia el acceso realizado, e incorporar variables de contexto y la perspectiva de los actores para comprender mejor el impacto de la reforma en el uso de servicios.Palabras Clave: Accesibilidad a los servicios de salud, prestación de atención de salud, Colombia, literatura de revisión como asunto (fuente: DECS, BIREME). Background This article presents a qualitative study, conducted in Chile and Colombia, on mental health care access and experiences with psychotherapy from the perspectives of economically disadvantaged clients. Current epidemiological data show it is important to conduct psychological interventions to address depression in poor populations, given the high prevalence of this disorder in these groups. However, despite efforts made in both countries at the health care policy level, the impact of the available psychological care has been weaker than expected. Aims Based on this observation, this study sought to collect more information for understanding this problem by incorporating patients’ views. Materials & Methods To shed light on this issue, 24 participants from Chile and Colombia were interviewed with respect to their opinions on access to psychotherapeutic care and their experience with it. Semi‐structured interviews conducted were analysed using Grounded Theory procedures. Results The facilitating and impeding factors that low‐income people have regarding access to psychotherapy, in an institutional context, and specific challenges involving changes in their understanding of their problems and their own role in recovery are presented. These changes involve distancing themselves from their original culturally‐shaped understanding of psychological disorders. Discussion The influence of psychotherapy as a force capable of shaping culturally determined meanings in clients from economically disadvantaged areas is problematised. Conclusion The results of this research may encourage clinicians to see the client's uniqueness within a certain social order and a social network as different from that of the therapist.
