The Anterior cruciate ligament (ACL) is a unique structure and one of the most important ligaments for knee stability, serving as primary restriction for the anterior tibial translation on the femur and secondary restriction to the knee external and internal rotation that is not sustaining weight. The objective of this study was to demonstrate the anatomy and biomechanics of anterior cruciate ligament as well as demonstrate the importance of the anterior cruciate ligament in the stability of the tibial-femoral joint. Literature review was performed using the data bases Scielo, Pubmed and Lilacs having as descriptors: "Anterior Cruciate Ligament", "LCA", "Anatomy" and "biomechanics" from the year 2008 to 2018. The LCA stability for the femorotibial joint and realization of the movement amplitude are in the ability of the anteromedial and posterolateral bands of the same in absorbing the entire load and traction of the joint when in antagonistic movements of the knee flexion and extension, favoring the stability of the tibial-femoral joint. Resumo O Ligamento Cruzado Anterior (LCA) é uma estrutura única e um dos mais importantes ligamentos para a estabilidade do joelho, servindo como restrição primária para a translação anterior da tíbia relativa ao fêmur e restrição secundária à rotação externa e interna do joelho que não está sustentando peso. Este estudo teve como objetivo demonstrar a anatomia e a biomecânica do ligamento cruzado anterior bem como demonstrar a importância do ligamento cruzado anterior na estabilidade da articulação tíbio-femoral. Foi realizado revisão da literatura usando as bases de dados Scielo, Lilacs e Pubmed tendo como descritores: “Ligamento Cruzado Anterior”, “LCA”, “Anatomia” e “biomecânica” a partir do ano de 2008 a 2018. A estabilidade do LCA para a articulação femorotibial e realização da amplitude de movimento estão na capacidade das bandas ântero-medial e póstero-lateral do mesmo em absorver toda a carga e tração da articulação quando em movimentos antagônicos de extensão e flexão do joelho, favorecendo na estabilidade da articulação tíbio-femoral. ObjectiveTo study the anatomy of the posterior cruciate ligament (PCL) and define anatomical parameters with the knee flexed at 90°.MethodsEight knees from cadavers were dissected in order to make measurements from the center of the anterolateral band to the roof (AL1), from the center of the anterolateral band to the anterior cartilage (AL2), from the center of the posteromedial band to the roof (PM1), from the center of the posteromedial band to the anterior cartilage (PM2), from the center of the tibial insertion to the medial region of the tibia (TIM), from the center of the tibial insertion to the lateral region of the tibia (TIL), from the center of the medial insertion to the medial meniscus (IMM) and the width of the origin of the PCL (WO). To obtain the results from each anatomical structure, the means and standard deviations of the measurements were calculated.ResultsThe measurements in millimeters that were found were AL1, 6.2; AL2, 4.9; PM1, 11.7; PM2, 5.5; TIM, 32.5; TIL, 40.6; IMM, 9.4; and WO, 32.5.ConclusionsThe PCL has an extensive origin. The center of the anterolateral band is 6 mm from the roof and 5 mm from the anterior cartilage of the knee. The tibial insertion is slightly medial and 10 mm distal to the posterior cornu of the medial meniscus.

Anatomical study of the positioning of guide wires for the reconstruction of the two bands of the anterior cruciate ligament using transtibial technique

PURPOSE: To determine whether it is possible and the difficulty of anatomical placement of guide wires for reconstruction of the two bands of the anterior cruciate ligament using transtibial technique and to measure the angle and positioning of the anatomic tibial guide wires for this technique. METHODS: Ten cadaver knees were dissected and a guide wire was positioned in the center of origin and insertion of each band. Were collected measurements of the distance from: 1) the entry point of the guide wire on tibial tuberosity; 2) the medial end of the tibia; 3) the tibial articular surface. Were also measured the medial and caudal angles of the guide wires. RESULTS: For the anteromedial band the medial angle was 19±5º and the caudal was 44±4º.The distances were 20±2 mm from anterior tuberosity, 26±5 mm from the tibial plateau and 25±5 mm from the medial end of the tibia. For the posterolateral band the medial angle was 26±5º and the caudal one was 36±8°. The distances were 31±6 mm for the anterior tuberosity, 24±4 mm for the tibial plateau and 17±7 mm from the end of the tibia. CONCLUSIONS: It is possible to position the guide wires of the two bands to reconstruct the anterior cruciate ligament using transtibial technique. The parameters for the proper positioning of the guide wire from anteromedial band is caudal angle of 44º with entry point 20 mm from anterior tuberosity; for the posterolateral band the caudal angle is 36º; from the tibial tuberosity the distance is 31 mm. This technique is difficult to perform leaving its questionable reproducibility.
