In this study, we aimed to examine the effect of L-arginine, tadalafil, and their combination for prevention of the ischemia reperfusion injury after testis torsion in rats. Methods: A total of 40 adult, male Sprague-Dawley rats were allocated into five groups. Three hours of left testicular torsion was performed in each group, excluding the control group. While the ischemia reperfusion (I/R) group had no treatment, I/R + Arg group received L-arginine, I/R + Td group received tadalafil and I/R + Arg + Td group received tadalafil and L-arginine 30 minutes before the detorsion. Then the left testis was untwisted for four hours of reperfusion. After bilateral orchiectomy, lipid peroxidation (LPx) and glutathione (GSH) activities were examined in testicular tissue. Spermatogenesis was evaluated with Johnsen's score. Results: LPx levels of the I/R group were found to be significantly higher than for groups that received drugs for both testes (p<0.001). GSH levels of the combination group were higher than I/R group in ipsilateral testis (p<0.01) and it was significantly higher than other groups for contralateral testis (p<0.001 for I/R group, p<0.01 for I/R + Arg, p<0.05 for I/R + Td). Mean Johnsen's score of the I/R group was found to be significantly lower than treatment groups in ipsilateral testis (p<0.001 for I/R + Arg + Td group, p<0.01 for other treatment goups) and contralateral testis (p<0.001). The mean Johnsen score of the combination group was significantly higher than that of other treatment groups in ipsilateral testis (p<0.05) and it was significantly higher than in the I/R + Td group in the contralateral testis (p<0.05). Conclusions: L-arginine, tadalafil, and combination of these two molecules showed protective effect against ischemia/reperfusion injury for both testes after unilateral testis torsion.

RESUMO:A Hipertensão Arterial Pulmonar (HAP) ou Hipertensão Pulmonar (HP) é uma doença progressiva, ainda pouco descrita na medicina veterinária, caracterizada por aumento da resistência vascular pulmonar e conseqüente aumento da pressão arterial pulmonar, resultando principalmente em disfunção ventricular direita e morte por insuficiência cardíaca na ausência de tratamento. Na medicina veterinária a HP geralmente é identificada durante a avaliação de animais com doença pulmonar crônica ou insuficiência cardíaca direita. Portanto, o risco de pacientes veterinários apresentarem HP é refletido pela probabilidade de apresentarem doenças pulmonares ou valvares crônicas. O diagnóstico é realizado por ecocardiografia Doppler ou cateterismo cardíaco e o tratamento visa a eliminação da causa primária e estabilização cardiovascular, melhora na oxigenação e redução da pressão na artéria pulmonar. This situation ends with progressive right ventricular dysfunction and death from heart failure if untreated. In Veterinary Medicine, PH normally is identified during the evaluation of animals with chronic pulmonary disease or right cardiac failure. Hence, the risk of veterinary patients show PH is reflected by the probability that they have to suffer from pulmonary or chronic cardiac valve diseases. The diagnostic is performed by Doppler echography or cardiac catheterism and treatment is based on primary cause elimination and cardiovascular stabilization, as well as oxygen improvement and pulmonary arterial pressure reduction.

Introduction:Pulmonary arterial hypertension (PAH), characterized by increased pulmonary artery pressures in the absence of elevated pulmonary venous pressures, is a progressive disease associated with reduced exercise capacity and increased mortality risk. Current treatments for PAH include nonspecific vasodilators, prostacyclin and related analogs, and endothelin receptor antagonists. Since phosphodiesterase type 5 is highly expressed in pulmonary vascular tissues, agents that selectively inhibit phosphodiesterase type 5 activity induce pulmonary arterial vasodilatation, and are being developed for the treatment of PAH.Aims:The purpose of this review is to evaluate the existing evidence for the use of tadalafil, a selective phosphodiesterase type 5 inhibitor, in PAH.Evidence review:Data from erectile dysfunction populations indicate that tadalafil is well tolerated with an elimination half-life of 17.5 hours. Small pilot studies in patients with PAH of mixed etiology demonstrate that tadalafil reduces pulmonary vascular resistance and is associated with improved clinical status. A multicenter, randomized, placebo-controlled clinical trial in patients with PAH is currently recruiting patients.Clinical potential:Based on existing studies of sildenafil, a related selective phosphodiesterase type 5 inhibitor in PAH, and the findings of initial pilot studies, tadalafil appears to have excellent potential to provide therapeutic benefit in patients with pulmonary hypertension. The long elimination half-life of tadalafil makes it suitable for once-daily dosing.
