Objective: To evaluate the factors related to the time of exclusive breastfeeding (EBF) in infants from Pará de Minas and Três Corações (Minas Gerais) and Brumado (Bahia), Brazil. Material and Methods: All mothers attended in public and private hospitals of these cities between June and December 2012 (n=156) were invited to answer an interview related to EBF. Data were collected: (1) at maternity ward; (2) at 4 and (3) 6 months of the infant's life. In (2) and (3), data were collected by phone. Data were recorded in the SPSS software, and descriptive analyses and association of exposure and outcome variables related to the practice of EBF were performed. The chi-square test was used, considering p <0.05. Results: The total of women was in (1) 156, (2) 104 and (3) 123, and the EBF rate was 50.0%, 39.0% and 4.8%, respectively. In (1) mothers with lower schooling level have opted for EBF more frequently. In (1), the use of bottle did not interfere in EBF; on the other hand, in (2) and (3), the use of bottle has decreased the frequency of EBF (p < 0.05). Conclusion: The ideal frequency of EBF recommended by WHO to the six months of the infant's life is not followed in the evaluated cities. In addition, EBF can be influenced by the use of bottle and maternal schooling level.

OBJECTIVE:To analyze time trends in the duration of exclusive breastfeeding and associated protection factors. METHODS:Prevalances for total breastfeeding and exclusive breastfeeding in a population of four-month-old infants were analyzed. Data were obtained from population surveys carried out in 1991, 1997, and 2006, with 935, 2081, and 1568 infants respectively. The data were retrieved from interviews answered by parents or guardians. The prevalences were analyzed by Poisson regression in regard to: environmental, behavioral and socioeconomic conditions, maternal variables, and biological factors of infant. RESULTS:The median duration of total breastfeeding rose from 89 days (1991) to 106 days (1997), and fi nally to 183 days (2006). The median duration of exclusive breastfeeding remained around 30 days between 1997 and 2006. In the multivariate analysis, from the ten variables studied, only maternal schooling and age, address, and female gender of infant were maintained in the fi nal explanatory model. CONCLUSIONS:Despite the significant increase in terms of total breastfeeding duration, the same did not occur with the duration of exclusive breastfeeding.

Risk Factors for Discontinuing Breastfeeding in Southern Brazil: A Survival Analysis

To identify risk factors for discontinuing breastfeeding during an infant's first year of life. A cohort study recruited mothers in a hospital in São Leopoldo, Brazil, which mainly serves the low-income population. In order to obtain socioeconomic, environmental, and behavioral information, face-to-face interviews with mothers were conducted after birth, and when their infants were 6 and 12 months old. The duration of breastfeeding was investigated at 6 and 12 months, and recorded separately for each month. Depressive symptoms were assessed using the Beck Depression Inventory. The multivariate model for predicting the discontinuation of breastfeeding, adjusted Kaplan-Meier survival curves and Cox regression were used. Of the 360 participants, 201 (55.8%) discontinued breastfeeding within the first 12 months. A multivariate Cox regression model revealed that symptoms of maternal depression (low levels: RR = 1.59, 95% CI 1.02-2.47; moderate to severe: RR = 2.03, 95% CI 1.35-3.01), bottle feeding (RR = 2.07, 95% CI 1.31-3.28) and pacifier use in the first month of life (RR = 3.12, 95% CI 2.13-4.57) were independently associated with the outcomes after adjusting for confounders. Breastfeeding cessation rates were lower for children who did not use bottle feeding or a pacifier in the first month of life and for the children whose mothers presented with minimal depression. Early pacifier use and bottle feeding must be strongly discouraged to support long-term breastfeeding. In addition, screening maternal depression at a primary care service can be a step forward in promoting a longer duration of breastfeeding.
