Objective: To evaluate the performance of rapid prescreening (RPS) and 100% rapid review (RR-100%) as internal quality control (IQC) methods assessed by outcome at colposcopy, histopathology and repeat cytopathology for cases with false-negative results on cervical cytopathology at routine screening (RS). Study Design: Out of 12,208 cytology smears analyzed, 900 were abnormal. Of these, 656 were identified at RS, and 244 were false-negative, with 90.2% identified at RPS and 57.4% at RR-100%. Of the 900 abnormal cases, 436 were submitted for additional testing. Results: Of the 244 women with cytopathological abnormalities identified only by the IQC methods, 114 had supplementary examinations: 35 were submitted for colposcopy, 22 for biopsy and 99 for repeat cytopathology. The sensitivity of RPS for the detection of abnormalities identified on colposcopy, histopathology and repeat cytopathology was 87.5% (95% CI 67.6-97.3), 82.4% (95% CI 56.6-96.2) and 95.7% (95% CI 85.2-99.5), respectively. The sensitivity of RR-100% was 54.2% (95% CI 32.8-74.4), 52.9% (95% CI 27.8-77.0) and 47.8% (95% CI 32.9-63.1), respectively. RPS was more sensitive than RR-100% when compared to the findings on colposcopy (p = 0.011) and repeat cytopathology (p = 0.000). When compared to colposcopy, histopathology and repeat cytopathology, the sensitivity of RS was 83.2% (95% CI 76.1-88.9), 85.7% (95% CI 78.1-91.5) and 73.3% (95% CI 66.0-79.7), respectively. Conclusion: RPS performed better than RR-100% when compared to the results of colposcopy and repeat cytopathology.

Saúde Unipar, Umuarama, v. 18 n. 1, p. 3-7, jan./ abr. 2014 RESUMO: O carcinoma de colo do útero é o segundo mais frequente em mulheres no Brasil, tendo acometido mais de 18.000 mulheres no ano de 2008. O presente estudo objetivou analisar e correlacionar os resultados obtidos nos laudos citológicos e histopatológicos, conforme a faixa etária das pacientes. Para tanto, metodologicamente foi realizado um estudo observacional transversal retrospectivo por meio de 106 prontuários de pacientes que apresentavam laudos citopatológicos e histopatológicos com atipias cervicais atendidas no período de janeiro de 2007 a dezembro de 2008 no Centro de Saúde da Mulher em Piripiri, Piauí, Brasil. Foram obtidos os seguintes resultados: quanto ao exame citológico os resultados mais frequentes foram os de ASCH e ASC/AG, correspondendo a respectivamente, 39,6% e 30%. Houve maior número de pacientes com laudo de ASC/AG e ASCH no intervalo de 25 a 34 anos, correspondendo a respectivamente, 13,2% e 17,9% destas lesões. Na histopatologia, 44 casos (41,5%) foram considerados LSIL, 53 casos (50%) foram diagnosticados com HSIL e 7 casos (6,6%) foram considerados como neoplasia maligna invasiva. Sendo assim, este estudo confirma a acuidade diagnóstica dos exames citológicos e histopatológicos, tendo em vista o grande número de lesões observadas, especialmente em pacientes jovens, destaca-se a importância de orientar práticas sexuais seguras, melhor controle na frequência de rastreamento e seguimento clínico destas pacientes. CYTOLOGY AND HISTOPATHOLOGY OF PATIENTS ASSISTED IN A WOMAN HEALTH CENTERABSTRACT: Cervical carcinoma is the second most common carcinomas among women in Brazil, having involved over 18,000 women in 2008. Analyze and correlate the most frequent cervical lesions observed in cytological and histological examinations. A cross -sectional observational study was retrospectively performed with 106 patients from a Woman Health Center in Piripiri, Piaui, Brazil, from January 2007 to December 2008, who presented atypical cytopathologic and histopathologic cervical findings. The cytologic results were more frequent in ASC and ASCH/AG, representing 39.6% and 30%, respectively. There was a greater number of patients with report of ASC/ AG and ASCH in the range of 25 to 34, representing respectively 13.2% and 17.9% of these lesions. Histopathologically, 44 cases (41.5%) were considered LSIL, 53 cases (50%) were diagnosed as HSIL and 7 cases (6.6%) were considered invasive malignancy. This study confirms the diagnostic accuracy of Pap smears, colposcopy and histopathology exams. Estudo epidemiológico e de concordância diagnóstica entre a citologia, colposcopia e histopatologia, em pacientes com diagnóstico de Papilomavírus Humano.Epidemiological study and diagnostic compatibility among cytology, colposcopy and histopathology in patients with Human Papillomavirus diagnosis. ABSTRACTThe diagnosis of intraepithelial lesions and carcinoma of the cervix is performed by combining three methods, namely cytology, colposcopy, and histology. The present study represents a retrospective and descriptive cross-sectional epidemiological study of patients' charts with cytological and colposcopic findings compatible with HPV infection, at the Potengi Clinic, in the year 2012 (Natal/RN). We analyzed 111 medical records of patients diagnosed with HPV and the data collected were age, menarche, the onset of sexual life, marital status, sexual partners, contraceptive methods used and results of cytological, colposcopic and histopathological exams. Patients had a mean age of 30 years, most of whom were married (71.2%), menarche at 12-year-old, and sexual debut at approximately 17 years, with a mean of 9.4 partners. About 38% of them use oral contraceptives, 37% do not use any method and only 10% use condoms. The concordance between cytology and colposcopy and between cytology and histology was very significant (rs=0,953; p<0,05). From the analysis of the present study, it can be concluded that the association of cytological, colposcopic and histological methods is more efficient in the diagnosis of lesions caused by viruses such as HPV and carcinoma and should, therefore, be used together. RESUMOO diagnóstico das lesões intraepiteliais e do carcinoma do colo uterino é realizado mediante a associação de três métodos, que são a citologia, colposcopia e histologia. O presente estudo representa uma pesquisa epidemiológica transversal retrospectiva e descritiva de prontuários de pacientes com achados citológicos e colposcópicos, compatíveis com a infecção por HPV, atendidas na clínica Potengi, no ano de 2012 (Natal, RN). Foram analisados 111 prontuários de pacientes diagnosticadas com HPV e os dados coletados foram idade, menarca, início da vida sexual, estado civil, parceiros sexuais, métodos contraceptivos utilizados e resultados de exames citológicos, colposcópicos e histopatológicos.
