Abstract: -Pineoblastomas are uncommon pineal tumors, which demonstrate rapid growing and poor prognosis. We report the case of a 43-year-old man with an enhancing pineal region mass, which showed restriction of the diffusion on diffusion-weighted (DW) MR images. The surgical biopsy defined the diagnosis of pineoblastoma and the therapy was initiated with radiation and chemotherapy. Three months later, the follow-up MR imaging showed areas suggestive of necrosis and the DW images demonstrate no significant areas of rest… Show more

“…1C) (36), thereby increasing the isotropic component in diffusion tensor imaging (DTI) (37). Some studies have shown that PBs can invade the surrounding brain parenchyma (38,39) and may disseminate through the cerebrospinal fluid (40,41). However, we were unable to find many studies showing radiologically infiltrative PBs, suggesting that PBs with invasiveness comparable to that of GBMs are rare.…”

“…12) [49] . Reports in adults with pineoblastoma outline the restricted water diffusion the lesions show due to their high cellularity in diffusion-weighted MRI images [50] . Pineoblastomas are given grade IV WHO classification, implying the highest degree of malignancy, usually show rapid recurrence and cerebrospinal dissemination, and are associated with a poor prognosis [2,51] .…”

“…Diffusion restriction was demonstrated in both cases that had diffusion-weighted images available. Diffusion restriction Gener,6 has been suggested as a possible distinguishing characteristic at diagnosis (9) and a possible tumor marker at posttreatment follow-up (13).…”
