Migraine is a common neurological disorder, prevalent in almost all over the world population and being considered as the 13 th major medical disorder among other major disorders across the globe, generally characterized with unilateral or bilateral headache, nausea and vomiting. In developing world, countries like Pakistan, most of the patients are not well aware of migraine headaches, as usually taken for simple headache due to no proper awareness. Usually it is not a contagious condition and most of the time it is recoded as episodic in nature. To assess the migraine, its frequency, symptoms, usual triggers and patients' perceptions, the present study was conducted. For this purpose a prospective study was carried out based on properly designed questionnaire. The data were collected from 7 educational institutes, including students of Islamia College Peshawar, University of Peshawar, Khyber Medical College, Engineering University Peshawar, Fatima Jinnah College Peshawar, Commerce College Peshawar and Agriculture University Peshawar. Part of data was also collected from patients visiting Khyber Teaching Hospital Peshawar for their headache problems. A total of 270 questionnaires were filled up. About 81 (30.00%) headache cases were recorded, fulfilling the International criteria of Headache. Out of 81 recorded cases, 31 (38.3%) patients were of migraine with aura and 50 (61.7%) were of migraine without aura condition. The prevalence of migraine in the study population was found to be 30.00%. The frequency of female migraine patients was higher i.e. 31 (34.1%) than the male sufferers 50 (27.9%). The present study clearly showed that the higher frequency (65.0%) of migraine exists in the age group of above 30 years age. A family history for migraine was found in migraine patients. Usually the condition was found to limit the routine activities of patients. The present study recorded that most of the migraine patients (40.2%) did not visit doctors at hospital and preferably go for self-medication, which really needs a public awareness in order to properly cope this neurological disorder, which may pose future medical complications as well.

-Objective: To analyze the demographic features of the population sample, the time of headache complaint until first consultation and the diagnosis of primary and secondary headaches. Method: 3 3 2 8 patients were analyzed re t rospectively and divided according to gender, age, race, school instruction, onset of headache until first consultation and diagnosis (ICHD-II, 2004). Results: Sex ratio (Female/Male) was 4:1, and the mean age was 40.7±15 years, without statistical diff e rences between sexes. Appro x i m a t e l y 65% of the patients were white and 55% had less than eight years of school instruction. Headache complaint until first consultation ranged from 1 to 5 years in 32.99% patients. The most prevalent diagnosis w e re migraine (37.98%), tension-type headache-TTH (22.65%) and cluster headache (2. 73%). C o n c l u s i o n :T h e re are few data on epidemiological features of headache clinic populations, mainly in developing countries. According to the literature, migraine was more frequent than TTH. It is noteworthy the low school instruction of this sample and time patient spent to seek for specialized attention. Hypnic headache syndrome was seen with an unusual frequency.KEY WORDS: headache, clinic-based study, epidemiology.Epidemiologia das cefaléias primárias e secundárias em um serviço terciário brasileiro. RESUMO -Objetivo:Analisar os aspectos demográficos da população em estudo, o tempo decorrido desde o início da queixa de cefaléia até a busca pelo serviço, assim como o diagnóstico das cefaléias primárias e secundárias. Método: 3328 pacientes foram analisados re t rospectivamente de acordo com sexo, idade, raça, escolaridade, tempo decorrido entre o início da cefaléia e a busca ao médico e diagnóstico (ICHD-II, 2 0 0 4 ) . Resultados: A razão Mulher/Homem foi 4:1, ambos com média de idade 40,7±15anos, não havendo diferença significativa entre os sexos. Aproximadamente 65% dos pacientes eram brancos e 55% tinha menos de 8 anos de escolaridade. A duração da queixa de cefaléia até a primeira consulta foi de 1 a 5 anos em 32,99% dos pacientes. Os diagnósticos mais prevalentes foram: migrânea (37,98%), cefaléia do tipo tensional-CTT (22,65%) e cefaléia em salvas (2,73%). Conclusão: Existem poucos dados epidemiológicos de pacientes atendidos em serviços terciários, principalmente em países subdesenvolvidos. De acordo com a literatura, a migrânea foi mais prevalente que a CTT. Também é relevante observar a baixa escolaridade da população, assim como o grande tempo de espera até a primeira consulta. Um número incomum de pacientes com síndrome de cefaléia hípnica foi observado na amostra.

-This is the first headache survey in the region of Vitória, ES Brazil. A high prevalence of headache sufferers was found (52.8%). Headache was more common among women (63.9%) and less common among people older than 55 years old. The type of professional activity was not related with the headache prevalence. The most frequent causal attribution was stress. Most headache sufferers are not under regular medical treatment (9%), and most of them use analgesic drugs without proper orientation. The most used compounds are combinations with caffeine (33%) and simple analgesics (52.3%).KEY WORDS: headache, prevalence, self medication. Epidemiologia da cefaléia em Vitória, Espírito SantoRESUMO -Este estudo é o primeiro a avaliar a prevalência da cefaléia na região de Vitória, ES. Demonstrouse alta prevalência de portadores de cefaléia (52,8%), com maior freqüência no sexo feminino (63,9%) e em pessoas com menos de 55 anos. Não houve diferença de prevalência em relação ao tipo de atividade profissional. O estresse foi apontado como o fator causal mais freqüente. Verificou-se que poucos portadores de cefaléia fazem acompanhamento médico regular (9%) e que a maioria usa medicamentos analgésicos sem orientação adequada, sendo os medicamentos mais utilizados combinações contendo cafeína (33%) e os analgésicos comuns (52,3%). PALAVRAS-CHAVE: cefaléia, prevalência, auto-medicação.The importance of primary headaches is related to the high frequency of headache disorders, considerable impact of headache on work and social activities, and to the consequences of a considerable consumption of drugs by headache sufferers Here we report the results of the first large epidemiological headache survey among inhabitants of the region of Great Vitória, Espírito Santo Brazil. This study aimed to identify the prevalence of headache symptoms, the causal attributions of headache, the medical assistance sought by the headache patients, and the pattern analgesic use reported by headache sufferers in this region of the country. METHODA questionnaire was administered by a group of medical students attending a specialized headache service. This questionnaire was randomly applied to Great Vitória region inhabitants, including the cities of Vitória, Vila Velha, Cariacica, and Serra. The interviewers have randomly selected people in public areas such as bus stations, schools, churches, supermarkets, banks, shopping centers, and sidewalks. Gender, age, and main occupation data were recorded. The individuals were grouped in three age groups: <30, 30-55, and >55 years. The occupation type was classified into the following categories: home, employed, liberal professional, unemployed, rested, or student. The prevalence of headache symptom was assessed asking if the individual consider him or herself as a headache sufferer. People answering no were not considered to have headache and no further questions were done.
