HTLV-I associated myelopathy in patients from Brazil and Iran: neurological manifestations and cerebrospinal fluid findings

Abstract: SUMMARY -We analysed sera and cerebrospinal fluid (CSF) of 20 cases of human T-cell lymphotropic virus type (HTLV-I) associated myelopathy/tropical spastic paraparesis (HAM/TSP) from Brazil and Iran and as controls, 16 Brazilian HTLVI seronegative individuals afflicted with other neurological diseases. It was observed in the HAM/TSP patients that: 1) all had an inflammatory reaction within the central nervous system (CNS); 2) 95% (19/20) showed oligoclonal bands reflecting intrathecal IgG synthesis; 3) 85% (17… Show more

“…These patients had no symptoms of measles, rubella or zoster infections. In three HAM/TSP patients, no intrathecal HTLV-I-antibody synthesis was detectable (Table 2) despite elevated HTLV-I antibody titres in blood 14 .…”

“…HAM/TSP does not exhibit a MRZ reaction, but as most other chronic infectious diseases, a local anti-HTLV-I response with ASI values above 1.5 (ASI = antibody specificity index). There is no correlation between CSF immunoglobulin or antibody levels and the clinical symptoms 14,15 . The CSF levels depend much more on the localization of the inflammatory lesions than on their inflammatory, destructive activities 2,3 .…”

“…28,29 The well-known variability in the progression of the disease may in part explain the lack of significant differences between groups regarding the duration of the disease. 28,30,31 Thus, duration does not necessarily implies greater disability.…”

“…A verificação de anticorpos totais ou específicos no LCR não permite demonstrar se estes são produzidos no SNC ou provêm do sangue pela passagem da barreira hemato-LCR (5,10,16) . Para tal é necessário relacionar os valores do LCR com o soro, considerando a função da barreira hemato-LCR.…”
