SummaryA 22‐year‐old Irish Sport Horse presented following a period of bilateral blindness with subsequent spontaneous resolution. Brain magnetic resonance imaging (MRI) demonstrated features consistent with pachymeningitis. Cerebrospinal fluid (CSF) analysis revealed increased numbers of basophils, suggestive of an immunologic or neoplastic aetiology. It was suspected that the enlarged meninges led to compression of the optic nerves in the optic canals, causing bilateral blindness. Initial spontaneous remission, as seen in this case, has been reported in human patients with this condition. Pachymeningitis should be considered as a possible cause of cranial nerve deficits, including bilateral and unilateral blindness, in horses.

RESUMO -As três espécies de esquistossoma podem comprometer o sistema nervoso. O S. mansoni é responsável pela esquistossomose no Brasil, sendo a mielopatia uma forma grave desta helmintose. O p ropósito deste trabalho é analisar as alterações do líquido cefalorraquidiano (LCR) para dar mais subsídios para o diagnóstico da esquistossomose raquimedular. Fizeram parte deste estudo 22 amostras de LCR de pacientes com esquistossomose espinal. Os resultados das análises destas amostras mostraram que a associação de alterações do LCR com quadro inflamatório e RIFI-IgM positiva ocorreu em 88% dos pacientes, que o eosinófilo esteve presente em apenas 7 amostras (36,8%), e que 3 dos 22 pacientes estudados a p resentaram LCR normal. Conclui-se que o exame de LCR é coadjuvante muito útil para o diagnóstico da neuroesquistossomose.PALAVRAS-CHAVE: líquido cefalorraquidiano, esquistossomose raquimedular.Cerebrospinal fluid in the diagnosis of spinal schistosomiasis ABSTRACT -Cere b rospinal fluid (CSF) changes in spinal shistosomiasis have been described. Its characteristic f e a t u res are mild to moderate pleocytosis, presence of eosinophils, slight to moderate protein incre a s e , elevated gamma globulin concentration and a positive immune assay. Nevertheless, these abnorm a l i t i e s a re not always present together and there f o re difficulties may arise in the assessment of the diagnosis. The purpose of this paper is to evaluate the importance of each CSF alteration concerning the diagnosis in 22 cases of spinal shistosomiasis. According to the results, only 20% of the cases had all the five feature that are considered to be characteristic of spinal shistomiasis. Abnormal cell count was present in 86%, p rotein increase in 77.3%, immunoglobulin G increase in 60,8%, eosinophils were present in 36.8% and i n d i rect fluorescent antibody test was positive in 68.2%. In three cases all CSF parameters studied were within the normal limits. We report the case of a 9-year-old girl was admitted to the hospital with fever, headache, and vomiting. Physical examination revealed stiff neck, positive Brudzinski's sign, negative Kerning's sign, and skin rash in lumbar region. In her past medical history, there were no relevant data except for food allergies and atopic dermatitis but no immune deficiency was identified.Computed tomography scan of brain showed no acute findings and the blood test showed normal inflammatory markers: C-reactive protein 2.40 mg/dL [Reference value (RV) 1-10 mg/dL] and procalcitonin 0.05 ng/mL (RV <0.1 ng/mL).Clinical examination and laboratory testing of the blood sample raised suspicion of viral meningitis, so a lumbar puncture was performed to obtain a sample of cerebrospinal fluid (CSF). In the laboratory, CSF examination also revealed normal protein and glucose concentrations, but the cell count study showed lymphocytic pleocytosis with 480 white blood cells/μL: 60% lymphocytes, 16% monocytes, 22% basophils, and 2% eosinophils. However, abnormal findings were not evidenced in peripheral blood (Figure 1).
