The Influence of End-Stage Liver Disease and Liver Transplantation on Thyroid Hormones

Abstract: -Background -Thyroid dysfunction has been reported in most chronic illnesses including severe liver disease. These defects in thyroid hormone metabolism result in the sick euthyroid syndrome, also known as low T3 syndrome. Objectives -Our objective was to evaluate the thyroid function in patients with end stage liver disease prior and after deceased donor liver transplantation and to correlate thyroid hormonal changes with the MELD score (Model for End stage Liver Disease). Methods -In a prospective study, ser… Show more

“…Although the relationship among hypothyroidism, cirrhosis, and HCC development has been emphasized in the literature, the issue is still controversial due to the different results obtained by different studies. [ 8 , 9 , 11 – 13 ] Our study demonstrated the presence of hypothyroidism in patients with cirrhosis and HCC. Interestingly, our study revealed that hypothyroidism levels were significantly different among patients with cirrhosis and HCC.…”

“…[ 7 ] Although some studies have found an inverse relationship between serum T3 concentrations and the severity of liver dysfunction, many others have not found clinical findings of hypothyroidism in patients with cirrhosis. [ 8 , 9 ]…”

“…Studies have demonstrated that thyroid dysfunction is associated with various conditions such as heart failure [ 12 ], cerebrovascular disorders [ 13 ], kidney disease [ 14 ], and liver cirrhosis [ 15 ]. However, it is still unclear if TSH level holds prognostic value for patients with HBV-related ACLF [ 15 , 24 , 25 ]. In this large-scale observational study of 1,862 patients with HBV-related ACLF, we found that TSH levels before treatment were correlated with reduced 30-day and 90-day survival, and that TSH level was an independent predictor of 30-day and 90-day outcomes.…”

“…However, one of the studies [ 23 ] only included 75 patients with ACLF, and in the other study [ 22 ] the follow-up period was only 30 days. In addition, other studies have indicated that TSH level has no predictive value on the outcomes of patients with advanced liver diseases [ 15 , 24 , 25 ]. Therefore, whether the level of TSH can predict the outcome of hepatitis B virus (HBV)-related acute-on-chronic liver failure (ACLF) is controversial.…”
