Percutaneous endoscopic gastrostomy for nutritional palliation of upper esophageal cancer unsuitable for esophageal stenting

Abstract: -Context -Esophageal cancer is often diagnosed at an advanced stage and has a poor prognosis. Most patients with advanced esophageal cancer have significant dysphagia that contributes to weight loss and malnutrition. Esophageal stenting is a widespread palliation approach, but unsuitable for cancers near the upper esophageal sphincter, were stents are poorly tolerated. Generally, guidelines do not support endoscopic gastrostomy in this clinical setting, but it may be the best option for nutritional support. Ob… Show more

“…Conversely, neurological dysphagia, the most frequent clinical indication for PEG in our hospital, seldom causes any technical problem that precludes the endoscopic approach. In patients with proximal esophageal cancer the endoscopic approach can be challenging, although in the authors' institution PEG was successfully performed in 22 out of 27 patients with this condition, in selected patients unsuitable to esophageal stenting (14). Nevertheless, SG (especially LG) may be the best option for patients without achievable endoscopy.…”

“…Moreover, in esophageal cancer patients with short life expectancy unsuitable for esophageal stenting, percutaneous endoscopic gastrostomy (PEG) may be a suitable means in order to achieve nutritional support, while allowing patients to be at home [116]. Endoscopically assisted nasogastric tube feeding is also a feasible palliative option for nutritional support, with a low complication rate.…”

“…Entretanto, sob acompanhamento e orientação individualizada, pacientes com câncer esofágico avançado, inoperável e sem perspectivas de retomada da ingestão oral, podem ser beneficiados com a introdução da gastrostomia endoscópica percutânea (PEG), levando em consideração que a maioria desses indivíduos sofre de disfagia significativa, perda ponderal e desnutrição, muitas vezes agravada pelo tratamento quimio e radioterápico 23 .…”
