Abstract: Celiac disease must be ruled out in patients with malabsorption complaints in or after interferon (or pegylated interferon) therapy. Screening for celiac disease with detection of anti-endomysium antibodies would be done in susceptible patients.

“…However, it is of note that this patient was AEA positive after IFN-alpha therapy, but his serology reversed to normal 1 year after stopping treatment, at which time he underwent intestinal biopsy which did not disclose a celiac morphology. In this context, several authors reported the onset of celiac disease during or after treatment with IFN-alpha for chronic hepatitis C [11][12][13][14][15][16][17]. Some of these cases were latent celiac patients, as suggested by positive celiac serology before IFN-alpha treatment.…”

“…Recently, considering that CD is an autoimmune-based entity and the hepatitis C virus (HCV) is suspected of being able to trigging autoimmune reactions, it has been hypothesized that HCV infection might predispose to CD [10] and that interferon-alpha (IFN-alpha) treatment might reveal it [11][12][13][14][15][16][17]. However, exhaustive analyses have not been performed in this field so far and no data are at present available on a possible relationship between CD and patterns of autoimmunity in HCV-infected populations.…”

“…These cases included a total of 22 patients, including the cases from our retrospective analysis. Of note, more than half of these cases were diagnosed after IFN-a therapy had commenced [26,[30][31][32][33] (see Table 4). We noted this in three of our patients.…”

“…There are reports of patients in whom treatment with interferon-α for hepatitis C led to celiac disease. [17][18][19][20][21][22][23][24] This suggests that interferon-α may precipitate the development of celiac disease in susceptible individuals. This is consistent with the known fact that interferon-α may precipitate or worsen autoimmunity in some individuals.…”
