Abstract: BACKGROUND: The rates of postoperative endoscopic recurrence (PER) in patients with Crohn’s disease (CD) are consistent. Anti-TNF therapy has been increasingly used in the postoperative setting, despite the lack of robust data in the literature on the measurement of trough levels and consequences of their use. OBJECTIVE: The aim of this review was to assess trough levels of infliximab (IFX) in CD patients after ileocolonic resections in correlation with the presence of PER. METHODS: We searched for studie… Show more

“…Since the early 90s, postoperative recurrence in CD has been the object of study in the IBD scenario. Anti-TNF therapy is being increasingly used in the postoperative setting in CD, however, data on the impact of serum concentrations in these patients on the presence of endoscopic recurrence are lacking [16]. Our cross-sectional study did not demonstrate a significant relation between low serum levels of IFX and postoperative endoscopic recurrence.…”

“…Our sample of patients was restricted and may have influenced the absence of a significant difference between the groups in relation to the primary outcome. However, it is worth mentioning that the only systematic review published on the subject showed that studies with serum concentrations of biologics in postoperative CD patients usually present a reduced number of patients [16]. Another limitation was the absence of centralization of colonoscopies, which may present a bias in the evaluation of the Rutgeerts’ score.…”

“…Even though there is appropriate evidence on the role of anti-TNFs in the prevention of endoscopic recurrence after ileocolonic resection, the effect of drug concentrations on recurrence rates has not been adequately explored and available data is scarce. A recent systematic review identified only four studies which assessed infliximab concentrations and endoscopic postoperative recurrence (POR) in CD, with higher concentrations mostly associated with lower POR rates [ 29 ].…”
