Identification of occult metastases of medullary thyroid carcinoma by calcitonin measurement in washout fluid from fine needle aspiration of cervical lymph node

Abstract: Medullary thyroid carcinoma (MTC) may occur sporadically or as a manifestation of an autosomal-dominant inherited syndrome, the multiple endocrine neoplasia type 2. DNA-based RET genotype analysis gained worldwide acceptance in the identification of asymptomatic gene carrier. MTC synthesize and secrete calcitonin, a well established tumor marker and postoperative level of serum calcitonin, indicates whether residual disease was left behind and whether reintervention is necessary. However, management is difficu… Show more

“…Another four additional studies were found by screening the references of these papers . Altogether 12 studies were included in our review . Papers were published from 2007 and 2014, one was multicenter (Italy‐Switzerland), nine were set as single‐center and retrospective (one from Switzerland, one from Japan, one from United States, and six from Italy), and two were case reports (one from Brazil and one from Turkey).…”

“…In such cases it is prudent to recommend serum calcitonin levels to establish the diagnosis of medullary carcinoma (138,144,145). Some authors have also suggested measuring calcitonin levels in the FNAB rinse to diagnose primary and recurrent/metastatic MTC as an adjunct to cytomorphology (146,147).…”

“…The measurement of calcitonin in the washout fluid has been recommended to improve the diagnostic sensitivity of FNAB for MTC (22,23) (B/C). In a recent retrospective study, the cytology detected MTC in 21/37 lesions with 56.8% sensitivity whereas the measurement of calcitonin in the washout fluid showed sensitivity and specificity rates of 100% using a cutoff of 36 pg/mL (24) (B).…”
