Objective: to establish the seroprevalence of Helicobacter pylori infection in children and to identify risk factors for seropositivity. Methods: a cross-sectional study established the seroprevalence of infection by Helicobacter pylori and afterwards a comparative study was performed amongst seropositive and seronegative children. A group of 228 children were cared for at the outpatient clinic of Instituto Materno Infantil de Pernambuco, from May to July 1999. Age, sex, indicators of environmental, social and economic conditions, health and nutritional status as well as children living in contact with dwellers with history of gastrointestinal pain or peptic disease were evaluated. All data was obtained through an interview with children's parents or caretakers. Nutritional status was evaluated by anthropometric index. Results: seroprevalence of infection caused by Helicobacter pylori was found in 32% (95% CI 26%-38%); 25.8% (95% CI 17.8%-33.8%) in preschool children and 39.4% (95% CI 30.4%-48.4%) in school age children. Analysis of environmental variables demonstrated higher frequency of seropositivity in children living in houses without toilets or sanitary flush toilet facilities (p=0.008), overcrowded (p=0.05), lower family income (p=0.03) and poor maternal education (p = 0.002). There were no statistically significant differences between the groups regarding health indicators, anthropometric indexes and children living with dwellers with history of gastritis or peptic ulcer. Conclusions: seroprevalence of Helicobacter pylori infection was high in the studied population, being even higher among the school age group. A positive association between seropositivity and less favorable environmental variables was detected.

Helicobacter pylori is a Gram negative bacterium that cause chronic gastritis, duodenal ulcers and can predispose the gastric cancer. The study aimed to determinate the prevalence of H. pylori infection by different methods of diagnosis in patients submitted to endoscopy. Of the 145 patients included in the study, were collected fragments of gastric mucosa for histological analysis, and for the rapid urease test. The breath test was also performed. The H. pylori infection was detected in 84 (57.9%) patients by histological study, the rapid test of urease was positive in 81 (55,8%) and the breath test in 62 (56,3%). There was no statistically significant difference when comparing the prevalence of infection by different methods of diagnosis. The prevalence of H. pylori infection in our community was lower than that found in the literature for patients with age similar to this study (mean = 53.19 years).

Clinical Outcomes of Eradication of Helicobacter pylori in Nonulcer Dyspepsia in a Population with a High Prevalence of Infection: Results of a 12-Month Randomized, Double Blind, Placebo-Controlled Study

Ninety-one Helicobacter pylori-positive patients with nonulcer dyspepsia were randomized to receive either lansoprazole, amoxicillin, and clarithromycin or lansoprazole and placebo. A validated questionnaire assessed dyspeptic symptoms at baseline and at 3, 6, and 12 months. Endoscopies and biopsies were performed at baseline and at 3 and 12 months. There was an overall trend, although not statistically significant, for a benefit of H. pylori eradication. Of the patients in the antibiotics group, 16 of 46 (35%) had symptomatic improvement, versus 9 of 43 (21%) in the control group (P = 0.164). In a secondary analysis, it was found that of the patients without endoscopic gastric erosions, 15 of 34 (44%) in the antibiotics group and 5 of 33 (15%) of controls had symptomatic improvement (P = 0.015). Helicobacter pylori eradication did not prove to be clinically beneficial, although a tendency to symptomatic benefit was detected. Further studies are necessary to confirm the implications of endoscopic gastric erosions in these patients.
