Objective To understand the effect of age on health‐related quality of life (HRQoL) in patients with hearing loss and determine how primary language mediates this relationship. Study Design Cross‐sectional study. Setting General otolaryngology clinic in Los Angeles. Methods Demographics, medical records, and HRQoL data of adult patients presenting with otology symptoms were reviewed. HRQoL was measured using the Short‐Form 6‐Dimension utility index. All patients underwent audiological testing. A path analysis was performed to generate a moderated path analysis with HRQoL as the primary outcome. Results This study included 255 patients (mean age = 54 years; 55% female; 27.8% did not speak English as a primary language). Age had a positive direct association with HRQoL (p < .001). However, the direction of this association was reversed by hearing loss. Older patients exhibited significantly worse hearing (p < .001), which was negatively associated with HRQoL (p < .05). Primary language moderated the relationship between age and hearing loss. Specifically, patients who did not speak English as a primary language had significantly worse hearing (p < .001) and therefore worse HRQoL (p < .01) than patients who spoke English as a primary language with hearing loss. Increasing age was associated with bilateral hearing loss compared to unilateral hearing loss (p < .001) and subsequently lower HRQoL (p < .001). Polypharmacy (p < .01) and female gender (p < .01) were significantly associated with lower HRQoL. Conclusion Among otolaryngology patients with otology symptoms, older age and not speaking English as a primary language were associated with worse hearing and subsequently lower HRQoL.

Presbiacusia é a perda auditiva característica da senescência que causa prejuízos na socialização do acometido. Objetivo: Investigar a presença de presbiacusia em indivíduos na faixa etária de 60 a 65 anos com queixas auditivas. Métodos: Estudo descritivo e retrospectivo de indivíduos com queixas auditivas, na faixa etária de 60 a 65 anos, atendidos no período de janeiro de 2015 a dezembro de 2018 no Hospital Geral de Fortaleza. Foram analisadas as seguintes variáveis: sexo, tipo e grau de perda de auditiva, lateralidade e curvas timpanométricas. Resultados: Dos 81 exames audiométricos analisados (60,50% do sexo feminino), observou-se, em ordem decrescente: perda auditiva bilateral (88,90%), curvas timpanométricas Tipo A (86,42%), perda auditiva sensorioneural (79,01%) e perda auditiva de grau leve (41,98%). Conclusão: A alta incidência de presbiacusia em indivíduos com queixas auditivas na faixa etária de 60 a 65 anos reforça a necessidade do desenvolvimento de políticas de saúde auditivas.

ImportanceHearing loss is associated with higher hospitalization risk among older adults. However, evidence on whether hearing aid use is associated with fewer hospitalizations among individuals with hearing loss remains limited.ObjectiveTo assess the association between audiometric hearing loss severity and hearing aid use and hospitalization.Design, Setting, and ParticipantsThis population-based cross-sectional study used audiometric and health care utilization data for respondents aged 65 years or older from 4 cycles of the National Health and Nutrition Examination Survey from 2005 to 2016. Data were analyzed from February 23, 2021, to March 22, 2022.ExposuresAudiometric hearing loss severity and participant-reported hearing aid use.Main Outcomes and MeasuresThe main outcome was respondent-reported hospitalization in the past 12 months. Multivariable logistic regression was performed to assess the association of hearing loss severity with hospitalization. To assess the association of hearing aid use with hospitalization, propensity score matching was performed with 2:1 nearest neighbor matching without replacement.ResultsOf 2060 respondents (mean [SD] age, 73.9 [5.9] years; 1045 [50.7%] male), 875 (42.5%) had normal hearing, 653 (31.7%) had mild hearing loss, 435 (21.1%) had moderate hearing loss, and 97 (4.7%) had severe to profound hearing loss. On multivariable analysis, moderate and severe hearing loss were associated with hospitalization (moderate hearing loss: odds ratio [OR], 1.50; 95% CI, 1.01-2.24; severe hearing loss: OR, 1.71; 95% CI, 1.03-2.84). Of 1185 respondents with at least mild hearing loss, 200 (16.9%) reported using a hearing aid. Propensity score–matched analysis showed that hearing aid use was not associated with hospitalization (OR, 1.17; 95% CI, 0.74-1.84), including among respondents with moderate or severe hearing loss (OR, 1.17; 95% CI, 0.71-1.92).Conclusions and RelevanceIn this cross-sectional study, hearing loss was associated with higher risk of hospitalization, but hearing aid use was not associated with a reduction in hospitalization risk in the population with hearing loss. The association of hearing aid use with hospitalization should be evaluated in larger prospective studies with reliable data on the frequency of hearing aid use.
