Clinical trials are a valuable study design for evaluating interventions when it is ethical and feasible for investigators to randomly allocate study animals to intervention groups. Researchers may choose to evaluate the comparative efficacy of intervention groups for their effect on outcomes that are relevant to the specific objectives of their trial. However, the results across multiple trials on the same intervention and with the same outcome should be considered when making decisions on whether to use an intervention, because the results of a single trial are subject to sampling error and do not reflect all biological variability. The objective of this review was to provide an overview of important concepts when selecting intervention groups and outcomes within a randomized controlled trial, and when building a body of evidence for intervention efficacy across multiple trials. Empirical evidence is presented to highlight that integrating and interpreting the efficacy of an intervention across trials is hindered by a lack of replication of interventions across trials. Inconsistency in the outcomes and their measurement among trials also limits the ability to build a body of evidence for the efficacy of interventions. The development of core outcome sets for specific topic areas in dairy science, updated as necessary, may improve consistency across trials and aid in the development of a body of evidence for evidence-based decision-making.

The reason of insufficient development of dairy farming is often connected with impaired reproductive capacity in cows, which leads to their infertility due to obstetric and gynecological pathology. The following common pathologies are registered: delayed placenta, subinvolution of uterus, cervicitis, vaginitis, uterine inversion. Metritis of cows is diagnosed most frequently. With the development of metritis there is a violation of motor and secretory function of the uterus, its morphostructure of tissues changes, which is manifested in destructive and atrophic changes of the integumentary and glandular epithelium. It is characterized by an increase of body temperature, decreased appetite, fatness. The cow often assumes a position to urinate, bends its back, and hunches for a long time. Inflammatory exudate is released from the genital slit. During the examination of the uterus enlargement of the horns, their asymmetry, fluctuations and pain are observed. Depending on the location of the process there are inflammations of the mucous membraneendometritis, middle muscular membranemyometritis, serousperimetritis and wide uterine ligamentsparametritis. Most often several or all layers of the uterus are involved in the inflammatory process. According to the course, they can be acute, subacute and chronic. By the nature of the manifestation the following ones are distinguished: clinical or subclinical; by the nature of the exudatecatarrhal, catarrhal purulent, fibrinous, necrotic and gangrenous. In addition, there are violations of the morphostructure of the ovaries, which are manifested by inferiority of intercourse cycles, or anaphrodisia. Laboratory tests of inflammatory exudate reveal sulphurcontaining substances that have increased proteolytic properties that lead to exposure and hyperplasia of the epithelium of the endometrium and glands, fusion and cyst formation, as well as the amino acid cysteine, which has a toxic effect on the contractile apparatus of sperm. In addition, exudate contains ammonia, hydrogen sulphide, hydrogen peroxide, which are toxic to sperm, eggs and embryos. Thus, structural and functional changes in the uterus and ovaries of cows in case of metritis lead to the loss of reproductive capacity in cows (infertility).

It is established that the physiological course of the gestational process occurs with the intensification of coagulation processes in the hemostasis system with thrombophilia, which has a two–phase nature – from the first month of pregnancy to the seventh and in the period of 2–3 days to calving with a peak of hypercoagulation by 2–5 that day after birth. The activation of hemostasis components is correlated with a high level of natural anticoagulant protein C and increased fibrinolysis activity through tissue plasminogen activator, which is controlled by a high level of its type I inhibitor. The main hemostasiological criteria are the levels: in phase I – fibrinogen 4,9–6,1 g/l, soluble fibrin about 0,04 g/l, activated partial thromboplastin time 38,1–39,7 s, functionally inactive forms prothrombin 0,88 ± 0,2 mg/ml in the first month of pregnancy; in phase II – fibrinogen 5,1–7,0 g/l, soluble fibrin about 0,01 g/l, functionally inactive forms of prothrombin 0,17 ± 0,05 mg/ml and protein C 72,0 ± 1,4 % after childbirth. In obstetric and gynecological pathology, the hypercoagulable state is aggravated to consumption coagulopathy, which is reflected in high plasma levels of soluble fibrin – 0,064 ± 0,005 g/l, fibrin / fibrinogen cleavage products – 7,5 ± 0,4 μg/ml, functionally inactive forms of prothrombi. – 1,90 ± 0,34 mg/ml and inhibitor of tissue plasminogen activator type I – 45,8 ± 0,9 IU/ml at the lowest level of tissue plasminogen activator – 0,38 ± 0,08 IU/ml (according to the norms 1,1–1,3 IU/ml) and protein C deficiency – <70%. Hypercoagulation in pregnant cows with purulent– necrotic lesions of the extremities is exacerbated by more pronounced hyperfibrinogenemia, an increase in its metabolites by 2,7 times (p<0,05) and a decrease in the activity of F XIII by 1,3 times (p<0,05) and the amount in the blood of proteinase inhibitor α2–M – 1,6 times (p<0,05). At the same time, the hypercoagulation syndrome in cows with obstetric and gynecological pathology due to its comorbidity with orthopedic not only intensifies, which is expressed in an increase in the level of hyperfibrinogenemia by 1,2 times (p<0,05), reduction of general coagulation tests by 1,3–2,1 times (p<0,001) and a decrease in the activity of FXIII in 1,2–1,3 (p<0,05), but is complicated by a decrease in the antiproteinase potential of the blood in 1,6–1,8 times.
