Background and purpose Respiratory dysfunction in Parkinson’s disease (PD) is often an underdiagnosed and untreated impairment associated with the disease. Clinically, a reactive approach to respiratory morbidity is taken, rather than preventative approaches that address underlying impairment/s. This systematic review identifies the current evidence to support nonpharmacological interventions to improve respiratory impairments in individuals with PD. Methods The relevant literature was searched using a customised and systematic strategy. Randomised and nonrandomised control trials of nonpharmacological interventions targeting respiratory outcome measures in PD were included. Outcomes of interest were respiratory morbidity and mortality, respiratory muscle strength, spirometry measures, lung volumes, peak cough flow, and perception of dyspnoea. Results Nonpharmacological interventions included: functional training, generalised strength training, respiratory muscle strength training, aerobic exercise, qigong, yoga, breath stacking, incentive spirometry and singing. Methodological quality of included studies varied. Meta‐analyses of nonpharmacological interventions demonstrated significant effects for inspiratory muscle strength (mean difference [MD] 19.68; confidence interval [CI] 8.49, 30.87; z = 3.45; p = 0.0006; I2 = 2%), expiratory muscle strength (MD 18.97; CI 7.79, 30.14; z = 3.33; p = 0.0009; I2 = 23%) and peak expiratory flow (MD 72.21; CI 31.19, 113.24; z = 3.45; p = 0.0006; I2 = 0%). Best‐evidence synthesis identified level 1 evidence supporting nonpharmacological interventions for improving peak cough flow and perceived dyspnoea. No studies were identified reporting outcomes of respiratory rate, inspiration:expiration ratio or respiratory morbidity or mortality in PD. Conclusions Nonpharmacological interventions improved respiratory muscle strength and peak expiratory flow in PD. Additional trials targeting respiratory dysfunction and longitudinal studies examining the relationship between respiratory dysfunction and morbidity and mortality rates in PD are required.

Objetivo: avaliar a mobilidade toracoabdominal em idosos com doença de Parkinson, antes e após realizarem treino funcional, bicicleta estacionária e exergame. Metodologia: 58 idosos foram randomizados em três grupos: um que realizou treino funcional (GF) n=18, outro que fez bicicleta estacionária (GB) n=20 e o terceiro que realizou o exergame (GE) n=20. A mobilidade foi mensurada pela cirtometria, por avaliador cego, em três níveis: axilar, xifoideano e umbilical, antes e após as intervenções. Resultados: não foram encontradas diferenças estatisticamente significativas no coeficiente de amplitude nos níveis axilar e xifoide e umbilical nos três grupos. No nível umbilical, entretanto, houve um discreto acréscimo, comparando antes e após as intervenções GF de 2,1(4,52) cm para 3,1(4,27) cm; GB de 1,1(3,16) cm para 1,7(4,68) cm e GE de 0,5 (3,29) cm para 1,0 (3,68) cm. Conclusão: os três programas de exercícios propostos não foram eficazes na melhora da mobilidade torácica no nível axilar e xifoide e parecem provocar uma pequena melhora na mobilidade abdominal, no contexto respiratório. AbstractIntroduction: elderly with Parkinson's disease (PD), have reduced thoracoabdominal mobility, which predisposes to respiratory complications such as aspiration pneumonia. Objective: to assess thoracoabdominal mobility in elderly with PD before and after performing functional training, stationary bike and exergaming. Methodology: fifty eight (58) elderly people were randomized into three groups: functional training (GF) n = 18, stationary bike (GB) n = 20 and exergaming (GE) n = 20. Mobility was measured by cirtometry, by blind assessor, in three levels: axillary, xiphoid and umbilical; before and after the interventions. Results: no statistically significant differences were found in the amplitude coefficient at axillary and xiphoid and umbilical levels in the three groups. At the umbilical level, however, there was a slight increase comparing before and after GF interventions from 2.1 (4.52) cm to 3.1 (4.27) cm; GB of 1.1 (3.16) cm to 1.7 (4.68) cm and GE of 0.5 (3.29) cm to 1.0 (3.68) cm. Problems in the respiratory system are the main cause of death in Parkinson’s disease (PD). Ventilatory limitations can also be part of a vicious cycle involving physical-functional limitations (e.g., walking difficulties) and the patients’ perception of fatigue. The objective of this study was to analyze the effects of an aquatic physical exercise intervention program on ventilatory parameters, perception of fatigue, and gait capacity in participants with PD. This quasi-experimental study had a single group with repeated measures in four assessments, proposing an aquatic physical exercise intervention program. The inclusion criteria encompassed being in levels 1 to 4 on the Hoehn and Yahr scale and having a medical certificate for the activities. Assessments took place at 3-month intervals between them—the first period was the control, the second following the intervention, and the third period was the follow-up. The intervention had 25 biweekly sessions over 3 months. A total of 13 people (71.3 ± 5.61 years old) participated in the intervention, without significant differences in the control period. Between the intervention assessments, they had statistically significant differences in MIP, MEP, FVC, Tiffeneau index, MVV, and fatigue. The study demonstrated that the aquatic physical exercise intervention was effective for ventilatory outcomes and fatigue in people with PD.
