Objective To assess the quality of the Primary Health Care services provided to children and the feasibility of using the Brazilian version of Primary Care Assessment Tool (PCAT-Brazil) as a routine quality assessment tool. Methods A cross-sectional study was carried out in Joanópolis, a small rural town in the State of São Paulo (SP), Brazil. Seven health professionals and 502 caretakers of children using the public health center were interviewed using the PCAT-Brazil, collecting data on the core and related attributes of Primary Health Care provided to children. The score of each attribute was calculated. Results Caretakers rated as good the following attributes; “degree of affiliation”, “first contact care − use of services”, “coordinated care”, and “comprehensive care − available services”. The attributes of “first contact accessibility”, “long term person care”, “comprehensive care − offered services” and “family- and community-oriented care” were scored as poor. The health professionals only rated the attribute of “first contact accessibility” as satisfactory, and considered that all other Primary Health Care attributes needed improvement. To conduct this study, at least 1,241 working hours were invested, and the estimated budget was R$12.900,00 (or U$3,953.00). Conclusion The use of the PCAT-Brazil as a routine assessment and planning tool seemed to be not feasible in the given setting due to high costs, lack of trained personnel and the huge workload. To overcome the encountered obstacles, advices are given based on field experience.

RESUMO Objetivo Investigar a evolução da cobertura da atenção primária à saúde (APS) nos municípios brasileiros de 2007 a 2016, no contexto da teoria da equidade reversa. Métodos Estudo ecológico realizado com dados de 5 564 municípios brasileiros. Os dados foram obtidos do Censo de 2010 e do sistema DATASUS. Os municípios foram classificados quanto ao seu desenvolvimento a partir de indicadores sociais selecionados, através de análise de cluster. Analisou-se a cobertura pela Estratégia Saúde da Família (ESF) nos grupos criados através da média e distância interquartílica, por análise de séries temporais e correlação entre variáveis. Resultados Foram criados dois agrupamentos de municípios, de condições mais (n = 3 293) ou menos favoráveis (n = 2 271). A diferença entre os grupos, para todos os indicadores avaliados, foi significativa ( P < 0,001). Em geral, independentemente do grupo, houve aumento na cobertura da ESF ao longo do período. Contudo, a partir de 2009, a cobertura média da ESF passou a ser maior no grupo com condições menos favoráveis. O aumento na cobertura também foi mais acelerado nesse grupo. Finalmente, uma mudança na correlação entre os indicadores e a cobertura de APS no primeiro e no último ano da série histórica indica que os critérios utilizados para expansão da cobertura no grupo com condições menos favoráveis passaram a valorizar a pobreza absoluta e não a iniquidade. Conclusões A APS no Brasil cumpre seu papel de política redutora de desigualdade de acesso. Portanto, não se aplicou a este caso a teoria da equidade reversa.

The article discusses the implications of health research in Primary Health Care (PHC). It analyses how the knowledge produced by the scientific investigation impacts the PHC services routine, emphasizing results production, dissemination, and use processes. Therefore, a participatory research of case study type, anchored in the constructivist theory, was carried out. PHC health professionals, managers and technicians of the Health Secretariat of the State of Ceará, Brazil, and the Municipal Health Secretariat of Fortaleza participated in the study. The hermeneutic-dialectical circle technique was used to collect information, and the empirical material analysis was based on critical hermeneutics. In the results, aspects related to the choice of research topics, relevance of investigations, and the use of the results for well-informed decision-making are discussed. To overcome the obstacles found in the production, dissemination, and use of the research results, we suggest a research priority agenda be developed at the municipal level, from which the problems of PHC would be listed, prioritized, and investigated based on a participatory methodology, capable of engaging all those involved.
