Objective: This article aims to discuss gestational diabetes mellitus (GDM), which is increasingly present during the gestational period. Elucidating about pathophysiology, epidemiology, etiology and prevention in order to spread knowledge in the academic and professional environment of the health area, amplifying the number of pregnant women diagnosed and, consequently, treated, to prevent the significant complications of this disease. Methods: In this work, an integrative literature review was carried out, using articles in the English language between the years 2010 to 2022, electronically searched in the free PubMed and UpToDate database, using the keywords: "Diabetes, Gestational", " Obesity, Maternal”, “Hypoglycemic Agents”, “Pregnancy Complications”, “Metabolic Diseases”. Results: Initially, 61 articles were identified, but 8 of these articles selected in the first instance did not meet the inclusion criteria, being excluded after analysis by the authors of this review article. In all 53 articles selected for the development of the work, they were explored in order to extract all kinds of relevant information and carry out counterpoints according to different dates and authors, resulting in a change and evolution of the methods of treatment and management of pregnant patients since 2010 up to the current scenario. Conclusion: This article made it possible to identify, through the authors' analysis, that GDM is an underreported pathology, in which pregnant women experience the repercussions of hyperglycemia without the correct treatment. However, when the diagnosis is performed properly, therefore, therapeutic measures are easy to manage.

Objective: To analyze the maternal-fetal characteristics of patients with gestational diabetes mellitus (GDM) attended in a private service. Materials and Methods: This is a cross-sectional study. Data collection was carried out from the medical records of patients diagnosed with GDM at Centro Hospitalar UNIMED, in Joinville city, between 2011 and 2017. Maternal-fetal characteristics, therapeutics and complications of pregnant women diagnosed with GDM were analyzed. Results: 515 patients with GDM were reported. Of the maternal characteristics, the mean age was (31.77), BMI (27.82). The proportion of obese pregnant women was (55.6%), normal (42.4%) and low weight (2%). The preference for cesarean section was (81.4%) and Gestational Age (GA) of GDM diagnosis (26.85), with higher diagnosis in the third trimester (65.2%) than second (29.3%) and first (5.3%). Incidences of hypertension (7.4%), preeclampsia (0.6%) and pregnant women with previous abortions (16.5%) were found. The mean glycemic profile showed HbA1C (5.26), estimated glucose (104.30) and FPG fasting (86.38), in the first hour (144.46) and in the second hour (64.15). Treatment with diet and exercise (38.3%), metformin (40%), insulin (14.5%) and combined (7.8%). Regarding newborns, the mean weight was (3.123.46) and the GA at birth (37.46). Regarding the percentile, AGP (71.8%), SGP (28.2%) and no cases of LGP. In terms of adverse outcomes, hyperbilirubinemia was evidenced in (25.6%), prematurity (10.9%) and fetal hypoglycemia (9.9%). Conclusion: Pregnant women diagnosed with GDM had a higher GA at diagnosis in the third trimester and more cesarean sections. Among the adverse outcomes related to newborns, there were more cases of hyperbilirubinemia, prematurity and fetal hypoglycemia.

Metformin versus glyburide in treatment and control of gestational diabetes mellitus: a systematic review with meta-analysis

Objective: To compare the major outcomes of use of metformin and glyburide in treatment of gestational diabetes mellitus. Methods: Studies published in English, in the last 10 years, in the databases MEDLINE ® , SciELO, LILACS and Cochrane Library were analyzed, and randomized controlled trials were selected. Health Sciences Descriptors were used to compose the search phrase, and the keywords "Gestational diabetes", "Glyburide", "Metformin" and their variations were searched in the Medical Subject Headings. PRISMA systematization was used to prepare this review, and a meta-analysis was conducted aiming to mathematically show the results of fasting blood glucose, postprandial blood glucose, birth weight and weight gain during pregnancy after using metformin and glyburide. Results: The studies evaluated birth weight, neonatal hypoglycemia, mode of delivery, need for intensive care, Apgar score, macrosomia, fasting glucose, postprandial glucose and weight gain during pregnancy. In 60% of studies, there were no statistically significant differences regarding safety and efficacy of administration of metformin and glyburide. Meta-analysis demonstrated the absence of statistical differences between these drugs in fasting blood glucose (p=0.821), postprandial blood glucose (p=0.217) and birth weight (p=0.194). However, significant differences were shown in weight gain during pregnancy (p=0.036). Conclusion: The methods are effective, but the adverse effects of glyburide are more common; therefore, the use of metformin should be recommended, if in monotherapy.
