Presence of dynapenia and association with anthropometric variables in cancer patients

Background Dynapenia is defined as an age-related loss of muscle strength. There is little information on dynapenia in cancer patients and on how it relates to anthropometric variables. The aim of this study was to analyze the presence of dynapenia and its association with anthropometric variables in hospitalized cancer patients. Methods Participants comprised adult and elderly cancer patients evaluated within the first 48 h of hospital admission to a tertiary public hospital, a referral center for gastrointestinal tract surgery. Anthropometric variables were measured according to standardized protocols. Dynapenia was identified based on handgrip strength (HGS), according to the cutoff points defined by the European Working Group on Sarcopenia in Older People (EWGSOP2), with values for women < 16 kg and for men < 27 kg. Statistical analysis was performed using SPSS software, version 22.0, with a significance level of 5%. Results This study included 158 patients aged in average 59.5 ± 14.0 years; of these, 53.6% were elderly, 58.9% non-white and 59.5% had some degree of malnutrition. The most prevalent type of cancer was that of the lower gastrointestinal tract (33.5%). The presence of dynapenia was observed in 23.4% of the patients and cachexia in 36.1%. There was an association between dynapenia with age (p < 0.001), life stage (p = 0.002) and race/color (p = 0.027), and also with body mass index (BMI) (p = 0.001) and adductor pollicis muscle thickness (APMT) of both hands (p < 0.05). After logistic regression analysis, adjusted for the sociodemographic variables, the APMT of the dominant hand and the low weight determined by body mass index remained associated with the occurrence of dynapenia (p < 0.05). Conclusions In this study we confirmed that dynapenia was present in cancer patients, being associated with APMT of the dominant hand and low weight. HSG was proven to be a reliable and complementary measure to be added to the process of assessing nutritional status, contributing to the nutritional diagnosis of these patients and to the detection of early muscle depletion.

Introdução: O estado nutricional em fase pré-tratamento pode estar relacionado a desfechos clínicos desfavoráveis em pacientes com câncer. Objetivo: Avaliar o estado nutricional de pacientes adultos e idosos com leucemia em fase de pré-tratamento oncológico. Método: Trata-se de um estudo transversal, retrospectivo, envolvendo pacientes com leucemia em fase pré-tratamento oncológico. Os critérios de inclusão foram ≥20 anos de idade; ambos os sexos; e matricula no Instituto Nacional de Câncer Jose Alencar Gomes da Silva (INCA). Os critérios de exclusão foram não ter diagnostico confirmado de leucemia; registro de avaliação do estado nutricional por meio da avaliação subjetiva global produzida pelo paciente; e não ter recebido tratamento oncológico prévio. Os testes qui-quadrado e t de Student foram utilizados. Resultados: Foram avaliados 69 pacientes com leucemia em fase pré-tratamento oncológico, sendo em sua maioria homens (52,2%) com menos de 60 anos de idade (52,2%). A prevalência de desnutrição foi de 65,2%, sendo mais expressiva nos idosos (78,8%, p=0,023). A prevalência de risco nutricional apresentou a mesma frequência (65,2%), no entanto, foi maior em pacientes com comorbidades (80,8%, p=0,017) e historia de tabagismo (90,9%, p=0,056). Conclusão: A maior parte dos pacientes com leucemia iniciou seu tratamento oncológico com o estado nutricional debilitado, o que se exacerbou entre aqueles com leucemia crônica, idosos, com comorbidades e historia de tabagismo.

Introdução: Nos últimos anos, o tratamento do câncer evoluiu, possibilitando uma maior sobrevida ao paciente, porém os efeitos colaterais, como a diminuição da imunidade e a fadiga, influenciam o sistema respiratório e muscular. Objetivo: Analisar a fadiga e as forças musculares respiratória e periférica em voluntários com câncer em tratamento e em indivíduos saudáveis. Método: Trata-se de um estudo analítico, observacional, transversal e controlado. Os indivíduos foram distribuídos em dois grupos: um grupo de câncer em quimioterapia e/ou em radioterapia (GCA: n=98; homens=35,72%; mulheres=64,28%; idade=58,13±13,26 anos; índice de massa corporal (IMC=26,23±4,04 kg/m2; tempo de diagnóstico de câncer=27,54±9,61 meses) e um grupo controle (GC: n=86; homens=30,23%; mulheres=69,77; idade=59,24±12,87 anos; IMC=26,76±4,04 kg/m2). Para todos os indivíduos, a fadiga relatada foi avaliada, usando-se a subescala de fadiga do questionário The Functional Assessment of Cancer Therapy-Fatigue (FACT-F). A avaliação das pressões respiratórias máximas foi realizada por meio da manovacuometria e da força de preensão palmar por intermédio da dinamometria manual. Resultados: O GCA apresentou maior índice de fadiga relatada (p<0,001; f2=0,382), valores inferiores para as variáveis respiratórias (PImax: p<0,001; f2=0,441; PEmax: p<0,001; f2=0,361), força de preensão palmar esquerda (p=0,024 f2=0,182), se comparado ao GC. Conclusão: Voluntários com câncer em quimioterapia e/ou em radioterapia apresentaram maiores níveis de fadiga relatada, com reduções da força muscular respiratória e da força de preensão palmar.
