Objective: To define the criteria for the continuity of care to elderly people submitted to arthroplasty. Method: This is a qualitative study, inserted in the constructivist paradigm, whose methodological option fell on research-action. The participants were the health professionals of an orthopedic service and of the community care teams in the area of the hospital. Results: The different techniques allowed us to identify the difficulties in the safe transition from the hospital to the community. At this level, two categories of criteria for continuity of care emerged: criteria associated with the risk of ineffective management of the therapeutic regimen, and criteria associated with the knowledge and level of competence of the informal caregiver. Final Considerations: An elderly person undergoing arthroplasty (hip or knee) has functional alterations that affect their capacity for self-care and may lead to dependence, our findings allowed the design of an algorithm to facilitate clinical decision making and promote a safe hospital-community transition.

BACKGROUND: A key dimension of a quality healthcare to patients is patient-centered care approach which is increasingly gaining recognition worldwide. However, this concept is not fully implemented in practice. AIM: The aim of this study is to provide outcomes from assessment of nurses’ perceptions about patient-centered care and the current trends in Primary Health Care system in South West Nigeria. METHODS: This study employed a qualitative participatory action research study approach and conducted a semi-structured individual interview with thirty-five nurses and four focus group discussions in Osun State South West Nigeria Primary Health Care centres. RESULTS: Primary Health Care (PHC) nurses perceived and described patient-centered care (PCC) as a global approach to improve and enhance nursing care to foster patient participation. The narratives were categorised into two: positive and negative perception. Ten main themes emerged: (I) Attitude of the nurses (ii) Lack of enforcement and implementation, (iii) Experience of the nurses, (iv) Quality-Caring, (v) Effective communication with patient, (vi) Motivated and Proactive healthcare, (vii) Sharpen the form of care, (viii) Outcome and after-effect driven healthcare, (ix) Approved support, and (x) Guarantor of service and motivation. CONCLUSION: Our participatory action research study on the assessment of nurses’ perception on the utilization of PCC at the PHC for effective and quality healthcare service revealed the importance of nurses’ role, acceptability of PCC and current nursing care practice at the Primary Health Care (PHC) rural community setting. Nurses as healthcare providers expressed PCC as a common and global approach that would enhance patient experiences and improves the quality of nursing healthcare delivery through integration of PCC and healthcare service at the PHC healthcare system.

Background: With increased research, the concept of people-centered care is advancing, and it remains an important concept in the health sector. Despite reported evidence of significant benefits, this concept is poorly utilized by professional nurses. The principle has not been demonstrated and absorbed at primary health care levels.Aim: The aim of this study is to present review of literature on people-centered care (PCC) in nursing and its practice in primary health care setting. Result:The findings of this research review outlined the evidence, practicability and acceptability of nursing care with people-centered care concept in the primary health care centers. Considering that nurses play a vital role in the health sector, it is possible to use people-centered care in Primary Health Care settings. However, there are many difficulties and obstacles identified such as: absence of nursing fraternity training and growth, absence of clear educational advancement resulting from change in accreditation, small number of nurses at the organizational level of health care organizations, and reduced global health care organization assistance. Discussion:The results from this review highlighted evidence of the relationship between quality healthcare deliveries of nurses with people-centered care concept in primary healthcare centers. Nurses' role in delivering PCC and gauging their knowledge and understanding is becoming imperative and crucial. In primary health care environments, nurses play a significant role in understanding a patient's culture of quality and efficient health care.
