A despeito da legislação Brasileira atual, que visa proteger e garantir os direitos da criança e do adolescente, a violência doméstica ainda é uma realidade presente no país. O presente artigo buscou analisar o perfil das famílias e do atendimento em saúde prestado pelo Núcleo de apoio à Estratégia Saúde da Família (Nasf) às crianças vítimas de violência doméstica no interior do Paraná. Trata-se de uma pesquisa descritiva documental, que analisou o perfil das famílias que possuem crianças vítimas de violência doméstica em seu meio e levantou a frequência de atendimentos prestados a essas crianças. Os resultados encontrados mostraram que a maior parte das famílias adota a violência como forma de resolução de problemas e que as crianças vítimas de violência raramente são identificadas nos atendimentos em saúde do município. Tais resultados indicam não só a necessidade do reforço da educação continuada para os profissionais de saúde, mas também de novas estratégias e técnicas que melhorem o desempenho dos profissionais de saúde no enfrentamento do problema.

Objectives To investigate the spatial and spatial‐temporal distribution of oral and maxillofacial injuries caused by urban violence, as well as to identify underlying disparities at regional level through a geostatistical approach. Methods This was a historical ecological cohort study of trauma cases caused by urban violence using aggregate data from victims assisted in a Brazilian medical‐forensic service between January 2012 and December 2015. The longitudinal patterns of change observed in each geographic area (neighbourhoods) were evaluated using the finite mixture model (FMM). The spatial autocorrelation of events was investigated using the Getis‐Ord Indicator (Gi*) to identify significant hot and cold spatial clusters. With a spatial regression model, it was also found when socioeconomic variables, residential infrastructure and neighbourhood infrastructure were associated with high incidence rates. The significance level was set at P ≤ 0.05. Results The finite mixture model revealed three different patterns of longitudinal trajectory of the incidence of oral and maxillofacial trauma caused by urban violence (TP1 to TP3, P < 0.05). TP1 was characterized by an incidence that remained stable and high over time, comprising 17.4% of the city's neighbourhoods. In TP2, it was observed that the incidence was moderate, with a slightly increasing trend in the last year evaluated, representing around 41.8% of the sample. In contrast, in TP3, it was found that the incidence was relatively low and remained stable over time, accounting for about 40.8% of the sample. The Getis‐Ord (Gi*) statistic identified significant high‐risk clusters in the western (P < 0.05), southern (P < 0.05), and eastern regions (P < 0.05) and low risk in the northern region (P < 0.05). The spatial regression model indicated significant association between areas with unfavourable socioeconomic conditions and higher incidence of events (β = 0.178, SE = 0.046, P < 0.001). Conclusions Clusters demarcating areas with high socio‐spatial vulnerability for urban violence and oral and maxillofacial injuries were identified. The findings highlight the need to improve living conditions in segregated urban areas and develop intersectoral actions to improve living conditions, employment, public safety, social support, health care and prevention.

Experiences of Violence Against Lesbian and Bisexual Women in Brazil

Background: Lesbophobia and biphobia are manifestations of homophobic violence directed at lesbian and bisexual women that results in daily violation of rights and social exclusion. Aim: To describe experiences of the violence against lesbian and bisexual women in Brazil. Methods: Sequential mixed methods study was carried out in 2 stages. In the first one, quantitative, an electronic questionnaire was applied to women from all regions, with questions about sociodemographic characteristics, self-identification and lesbophobic and biphobic events. In the second one, qualitative, lesbian, and bisexual women were interviewed face to face about the violence suffered. Outcomes: The chi-square test was applied to compare violence against lesbian and bisexual women (type of violence, place of aggression, gender and age range of the aggressor, bond with the aggressor, repetition of violence, and denunciation of violence) and the content analysis for qualitative data (main categories of analysis were events of violence, denunciation, and consequences of violence). Results: The report of violence was present in 65% of the answers. There was a predominance of psychological violence (39.8%), in the public environment (63%), practiced by men (73.2%), by strangers (66.2%) and repeatedly (82%). Lesbian women, compared to bisexuals, were more prone to violence in the public environment (59.5% vs 39.5%) and with repetition (84.3% vs 60.6%). The narratives explained intimidating experiences in the family environment (insults, threat of suicide or homicide and false imprisonment) and public (harassment, beatings, and rape). Discriminatory attitudes, insults, and refusal of service in restaurants and bars were recurrent. Clinical Implications: The data provide information that can serve to improve policy initiatives to reduce these episodes. Strengths and Limitations: This is the first study of mixed methods, with national coverage, on lesbophobia and biphobia events in Brazil. Future studies should include women underrepresented in this research as trans women, non-white, less educated, and from the most distant regions of the country. Conclusion: Lesbophobic or biphobic event has harmful repercussions for multiple aspects of these women's lives, including mental health.
