This article analyses the main characteristics of federalism in Brazil and its institutional relations with health policy. It discusses federalism from a classical perspective highlighting the essentially centralized nature of Brazil's system and the prevalence of decentralizing health policies underpinned by the principles enshrined by the 1988 Constitution.We used primary data obtained from an electronic questionnaire responded by secretaries of health sitting on the governing bodies of the country's health region and secondary data Ministry of Health databases covering the current health regions. The findings show that significant progress has been made in the implementation of regional governing bodies, yet without any significant impact on the reduction of deep regional inequalities in primary and hospital care. It concludes by suggesting that the persistence of inequalities is down to weak central coordination capacity and an inappropriate trade-off between a centralized federal system and competition between entities, thus undermining cooperative regionalization of the public health system as envisaged by the 1988 Constitution.

A proposta da territorialidade em saúde foi apresentada com o objetivo de direcionar as ações e políticas públicas que seriam aplicadas, de acordo com as necessidades de cada comunidade assistida pelos serviços de saúde. Assim, inserido nesse tema, essa pesquisa teve como objetivo investigar quais aspectos dificultam o acesso da população ao sistema de saúde público no Brasil, bem como as potencialidades existente capazes de melhorar esse acesso. Para tanto, a metodologia adotada consistiu na realização de uma revisão bibliográfica a partir de estudos coletados em bases de dados virtuais e submetidos a critérios de elegibilidade previamente estabelecidos, quais foram o ano de publicação entre 2015 e 2020, conteúdo pertinente e em língua portuguesa, e disponibilizados de forma gratuita e integral. Os resultados demonstraram que o Sistema Único de Saúde tem tentado adequar seu funcionamento e seus serviços de acordo com a necessidade da população, no entanto, ainda enfrenta inúmeros desafios de natureza, principalmente, burocrática e econômica, mas que busca superar por meio da adequação frequente da elaboração de suas ações e da participação popular como ferramenta de identificação das necessidades da população. Todavia, conclusivamente, é possível afirmar que o modelo existente atualmente ainda apresenta diversas deficiências, que estão interligadas com diversos fatores. Logo, é necessário que ainda debata-se bastante sobre o tema e novas políticas sejam elaboradas e implementadas, até que se alcance a otimização do sistema e seus serviços.

The study on the Local Health Councils in the city of São Paulo, carried out by Coelho (), is interesting in that it identified the following three key factors, acting simultaneously, that seem to improve the representativeness and the democratic legitimacy of the participatory spaces: the commitment of public officers to the project of citizens’ participation, the presence of a mobilized civil society, and some knowledge about how to organize participatory institutions.…”

This case study analyzed citizen participation at the Municipal Health Council (MHC) of Ribeirão Preto, in the state of São Paulo (Brazil), using a qualitative approach with a phenomenological basis and methodological triangulation: (i) analysis of the minutes of the meetings, (ii) passive observation, and (iii) semi‐structured interviews with councillors. The interviews were analyzed using content analysis and categorized according to three themes: (i) challenges and limitations of participation in the MHC, (ii) representativeness of the councillors, (iii) the Council and its influence on local health‐care policies. Problems with infrastructure and the organizational logistics of the MHC, as well as relational difficulties among members occupying varied roles and having different levels of knowledge about health, influenced the quality of the deliberative process. In contrast to studies that indicate poor participation by health‐care service users in spaces of dialogue around health‐care policies, this study found the users actively engaged and committed.
