Objective: To identify the most prescribed drugs by enteral feeding tube in a public hospital and to a guideline for safe medication preparation and administration via enteral feeding tubes. Methods: A cross-sectional study with analysis of the daily prescriptions of patients exclusively using an enteral catheter in intensive care units and inpatients of the medical clinic specialty, in which descriptive statistics were used to identify the prevalence of medication use by tube. In a second step, a protocol was developed for the situation in which oral medications are prescribed to patients with a tube in the digestive tract. Results: A total of 1.810 medications prescribed by enteral feeding tube, of these 1.810, 291 (16%) were prescribed in liquid pharmaceutical form (suspension, solution, syrup) e 1.519 (84%) were prescribed in solid pharmaceutical form (tablets, capsule). 52 medications were selected for the protocol, and 14 of them (26.9%) with the recommendation of not crushing and administering via enteral feeding tube. Conclusion: Our results highlights the performance of clinical pharmacist in monitoring patients with enteral feeding tubes and guidance of multi-professional team. The protocol elaboration will guide for preparation and administration medication through the enteral feeding tube, reducing possible adverse events and ensuring the safety and efficacy of drug therapy.

Cultura de segurança do paciente em hospitais públicos de ensino: estudo comparativo

Objetivo: comparar a cultura de segurança do paciente entre equipes de enfermagem de quatro hospitais de ensino públicos do Paraná, Brasil. Método: estudo transversal, descritivo, analítico, realizado em amostra representativa proporcional, aprovado pelo Comitê de Ética em Pequisa. Utilizou-se a versão eletrônica do instrumento Hospital Survey on Patient Safety Culture para a coleta de dados, analisados por meio de estatística descritiva e inferencial, com uso do Software Statistica Single User versão 13. As dimensões da cultura foram classificadas em forte, potencial e frágil; as diferenças foram determinadas pelo teste qui-quadrado e nível de significância p<0,05. Resultados: entre os 376 participantes foram constatadas diferenças significativas nas dimensões Frequência de eventos notificados, Percepção de segurança, Abertura para comunicações e Apoio da gerência. Aprendizagem organizacional/melhora continuada foi a única dimensão forte identificada em todos os hospitais. Conclusão: embora observadas diferenças entre hospitais, a cultura de segurança mostra-se frágil e representa barreira à qualidade assistencial.

Objective: This work sought to identify the innovative methodologies used to teach patient safety in undergraduate Nursing. Materials and method: this is a scoping review conducted according to the recommendations by the Joanna Briggs Institute Reviewers’ Manual, through the databases: Medline/PubMed, Cumulative Index of Nursing and Allied Health, Scopus, Web of Science, Education Resources Information Center, Latin American and Caribbean Literature on Health Sciences, Catálogo de Tesis de la Coordenação de Aperfeiçoamento de Pessoal de Nível Superior, The National Library of Australia’s Academic Archive Online, Digital Access to Research Theses Europe E-Theses Portal, Electronic Theses Online Service, Repositório Científico de Acesso Aberto de Portugal, National ETD Portal, and Theses Canada. Results: the study included 19 studies, most of descriptive type (n = 8; 42.1 %) and quasi-experimental (n = 7; 36.8 %) with quantative approach (n = 11; 57.9 %), conducted in the United States (n = 4; 21.1 %), in 2013 (n = 4; 21.1 %) and 2017 (n = 4; 21.1 %). The innovative methodologies used to teach patient safety in undergraduate Nursing that were highlighted included the scenario of the care practice simulated in the laboratory (n = 14; 73.7 %) and educational videos (n = 7; 36.8 %). Conclusions: it was identified that innovative methodologies used to teach patient safety in undergraduate Nursing were simulation, videos, staging/role playing, and films, all applied in the classroom teaching modality.
