Demonstration of Re-entry within the His-Purkinje System in Man

Abstract: SUMMARYRe-entry within the His-Purkinje system (HPS) was consistently observed in 15/24 consecutive patients in whom retrograde refractory period studies were performed using His bundle electrograms and the ventricular extrastimulus method. Within a narrow range of ventricular coupling intervals (V,V2), V, retrogradely conducted to the bundle of His (H2) with significant infra-His bundle conduction delay (V2H2 interval). At critical V2H2 delays another beat of ventricular origin (V3) followed V2 and was associ… Show more

“…However, in all instances, the retrograde His bundle deflection (H2) emerged from the local V2 electrogram as the S1S2 interval was shortened, and there were measurable and progressive retrograde delays in the His-Purkinje system (S2H2). In three patients, an S3 (patients 5, 11, and 15), and in only one patient (patient 1 (Table 4). As indicated above, another QRS morphology with a typical right bundle branch block appearance was also observed in three patients.…”

“…It too represents bundle branch reentry, but with an activation sequence opposite to that of the classic laboratory-induced type. [17][18][19] In other figure 3). Both the accessory pathway (AP) and normal pathway are depicted.…”

“…Macroreentry HPS (12,13). 10 of the 14 cases (3-9, 11, 13, and 14, Table I) demonstrated occurrence of macroreentry HPS in the form of V3 following retrograde H2 activation.…”

“…Despite the above reasoning, questions can be raised as to whether the V2 might still have blocked in the AV node rather than HPS, and whether the H2 deflection was either not identifiable for some reason or was obscured by the local V2 electrogram. These questions can be dismissed for the following reasons: (a) Once the retrograde H2 deflection emerges, the V2H2 intervals generally lengthen (rather than shorten) at shorter V1V2 (9,12,14,15). (b) The occurrence of gap phenomenon in most cases suggests retrograde H2 was recordable whenever the His bundle was depolarized.…”
