Abstract: Summary. Community samples of 673 subjects who had had serum vitamin B12 concentrations measured in 1958 were followed‐up 10 yr later. 134 of these subjects had died; the death rates showed no consistent trend in relation to the original serum vitamin B12 concentrations. Comparisons of concentrations in 1958 and 1968 in 480 subjects showed correlation coefficients (r) of between 0.46 and 0.71 in various subgroups. The oldest age group had a slightly lower mean concentration, and a larger proportion of this gr… Show more

“…43 However, the course of subclinical deficiency often appears stationary. [61][62][63] The biochemical abnormalities themselves frequently fluctuate 64,65 ; mildly to moderately elevated MMA levels improved spontaneously in 44% of 432 cases retested 1 to 4 years later, and only 16% worsened. 64 Those observations notwithstanding, the impulse for medical intervention is such that controlled cobalamin trials in subclinical deficiency have been called unethical by some investigators.…”

“…As a result of these IF-unrelated causes, progression of subclinical deficiency is often very slow [2,15]. Transformation of subclinical into clinically expressed deficiency is uncommon even after a decade, and even the biochemical abnormalities themselves often remain stationary or reverse spontaneously [2,16,17]. Fortification with vitamin B 12 may have its major impact on subclinical deficiency, which accounts for most of the new influx of cases of vitamin B 12 deficiency and which usually arises from nonmalabsorptive or weakly malabsorptive causes.…”

“…Such doses resemble the oral 1,000-to 2,000-µg vitamin B 12 doses required to treat patients with pernicious anemia and other diseases with failure of the IF system [5,6]. These unexplained but consistent observations suggest that small, heretofore standard oral doses [2,[15][16][17]. Second, the decision to dispense with untreated control groups surrendered the opportunity to document the extent of the spontaneous improvement that often occurs.…”

“…Hyams reported a sufficient secretion of IF in the gastric juice in the aged (7). The serum level of B12 was found to be low in the aged by others (8)(9)(10)(11)(12)(13).…”
